    Job summary

    The post holder will play a key role across the CHC service, working within the remit of the national framework for Continuing Healthcare. To deliver an effective, efficient, and equitable service to the people and population of Nottinghamshire.

    Prior knowledge of CHC would be beneficial but it is not essential for this role as you will work within an experienced and supportive team who will support you to develop the relevant knowledge required. However, the post holder should have experience of working within a health or care setting and will need excellent verbal and written communication skills as you will be required to work closely with patients, families, carers and other external partners.

    This role is a part time position working hours per week.

    Main duties of the job

    As a practitioner, your role includes completing reviews for nursing home residents who are in receipt of Joint Funding or Funded Nursing Care. You will work closely with nursing home staff or in patients homes to ensure reviews are undertaken in a timely fashion and ensuring a good standard of care is being delivered.

    Job responsibilities

    Job Purpose

  • To arrange and review patients in receipt of Funded Nursing Care (FNC) and Joint Funded (JF) reviews.
  • To work as an autonomous nursing practitioner within a multi agency setting in the provision of specialist advice.
  • To provide support across Nottinghamshire and neighbouring ICBs and Local authorities in the provision of health care assessment as required.
  • Act as an advocate for patient and carers in complex and vulnerable client group in partnership with Local authorities and health care providers to ensure that client needs are met in the appropriate setting.

  • Key Responsibilities

    Clinical

  • To work autonomously, as a health professional, in a local authority or health setting, to provide professionally accountable decisions and reports regarding clients eligibility for NHS Funded Nursing Care.
  • Responsible for carrying out health needs assessments and reviews for NHS Funded Nursing Care Contributions
  • Coordination of review process, making sound and robust recommendations.
  • Analysing information from a range of assessments and information sources to ensure the health and well-being of the client is optimised.
  • Complete NHS Funded Nursing Care reviews in a timely manner.
  • Contribute to social work assessments to identify the nursing needs of clients with Joint Funded care package/placements or those applying for long term care.
  • Continually review and improve systems which support the on-going monitoring, and review of client placements to ensure nursing needs are being met.
  • Provide information and advice, as necessary to lead clinicians, Local Authority Care Managers, Care Home Managers and other professional groups.
  • Work in partnership with social work colleagues and to advise on the most appropriate setting to address clients changing health and well-being needs.
  • Monitor the provision of evidence based/best practice.
  • Proactively raise issues and provide solutions in partnership with other colleagues in relation to change management, working across agencies to enhance care pathways and patient/carer experience.

  • To be responsible for the development of own knowledge and clinical practice and support ongoing professional development of team members.
  • Participate in the development of the teams clinical knowledge and specialist expertise.

  • Contribute to the implementation of service and team developments.
  • Utilise enhanced skills and knowledge to promote continuous service improvements.
  • Support the Continuing Care Service Head in change management and continual improvement of the service delivery.
  • Contribute to the Clinical Governance agenda and apply risk management systems.
  • Participate in relevant research and audit activity.
  • Participate in peer review.
  • Team Working

  • Support and initiate effective team working both across and within Health and Social Care Teams.
  • Work closely with Continuing Care Service Head and colleagues to agree a consistent approach to implementation of the NHS Funded Nursing Care policies.
  • Support the motivation of team members and assist in the implementation of change management process.
  • Patient and Public Involvement

  • Actively encourage patient and carer involvement in health needs assessments and at relevant multi-disciplinary meetings in order to complete reviews. To seek feedback about the service provided.
  • Help identify pathways of communication in the long term and continuing care process for patients and carers.
  • Record Keeping

  • Promote and maintain good practice in record keeping by self and the team, to ensure professional standards are met.
  • Complete electronic records of all Health Needs Assessments.
  • Keep a record of all activity, as agreed with the Continuing Care Service Head.
  • Produce professional reports for presentation at multi-disciplinary and inter agency meetings.
  • Participate in audit of assessments and standard setting.
  • Ensure adherence to Caldicott guidelines and data protection act.
  • Health, Safety and Security

  • Promote and maintain best practice that ensures the health, safety and security of self, patients and colleagues in accordance with the policies, procedures and guidelines of Nottingham CityCare Partnership.
  • Practice in accordance with procedures and guidelines for the protection of vulnerable adults and child protection.
