    Job summary

    Stockport diabetes is delighted to be successful in being selected as a site for the newly launched national Transition and Young Adult Care (TYA) Project Pilot, with all the exciting opportunities and potential this brings. This national project is part of the joint NHS Diabetes and CYP Transformation Programme to improve care and outcomes for children and young adults, living with diabetes, age years. The role is until March 2025. Key areas of focus for this programme are improving care for those transitioning from paediatric to adult care and addressing the poorer outcomes for children and young adults with diabetes at transition a Pilot Site we have received funding to enable us to build the evidence base with regards to providing clinically effective TYA Service Specification for this TYA Pilot is a clinically led consensus approved by the National Children, Young Adults and Diabetes Oversight Group. The national vision for the pilots is that this model service specification will be implemented by all sites as a minimum is the welcome expectation we develop innovative models of care; guided by patient led feedback and supported by different disciplines such as psychology practitioners and youth achieve the aims of the Pilot Project we are recruiting dynamic, enthusiastic and forward-thinking multidisciplinary professionals who have experience and knowledge of working with young adults with diabetes.

    Main duties of the job

    To be a key member of the Diabetes TYA Pilot Project, to contribute specialist diabetes dietetic advice and support, to work dynamically and innovatively with the TYA professionals to achieve the Project's aims.

    To provide an efficient, effective and consistently high-quality Nutrition and Dietetic service to TYA people with diabetes at Stockport Foundation Trust. The role will focus on providing a specialist Diabetes TYA dietetic service to meet the needs of TYA patients with complex acute and chronic disease processes.

    To provide evidence-based care, education and information to patients, carers and health care professionals. This will be in a range of inpatient, outpatient and community settings.

    Job responsibilities

  • To provide a specialist diabetes nutrition therapy service to the Diabetes TYA Pilot Project, providing advice and support to these professionals and patients
  • To dynamically contribute to the Diabetes TYA Pilot Project, including contributing to developing innovative models of care and ways of working, to achieve the Projects aims and meet the needs of this patient group.
  • To effectively communicate with the specialist paediatric diabetes dietitians to facilitate an effective and smooth transition of young people with diabetes
  • Provide a professional, evidence-based nutrition therapy service to designated adult wards and outpatient clinics at Stepping Hill and in community settings in Stockport.
  • Act as an expert resource on nutrition science and therapy for staff and patients.
  • Provide training of healthcare professionals regarding in relation to nutritional care and interventions, including presentations.
  • Liaise with medical, nursing staff and other healthcare professionals.
  • Participate in relevant ward rounds and meetings.
  • Contribute to developing diet sheets and new teaching materials.
  • Collect all data necessary to meet local requirements for service monitoring.
  • Participate in Quality Assurance programmes and audit.
  • Maintain professional competence and expertise, keeping up-to-date with current research, working practices and models of working.
  • Participate in personal and professional development through objective setting and regular job review.
  • Work as a committed and effective member of the Integrated Nutrition and Dietetics team and TYA Diabetes team in order to maximise the potential of the service and profession.
  • Work towards gaining experience in a specialised area of dietetics if service demands require this, by shadowing and providing cover when appropriate.
  • Observe the provisions of, and adhere to, all Trust policies and procedures.
  • Comply with professional guidelines and Code of Practice.
  • Work with the multidisciplinary diabetes teams to manage patients on insulin pumps and continuous glucose monitoring.
