An exciting opportunity has arisen for a Band 5 registered nurse to work within a Community Treatment Team based in Gateshead.
The Gateshead West Community Treatment Team is a multidisciplinary service which provides assessment and treatment for people experiencing moderate to severe mental health problems.
Your role will incorporate working closely with partner agencies/services in delivering effective care and treatment.
We strongly value supervision and support clinicians to develop their clinical skills. Whilst working in the team you will have access to a bespoke training programme which supports clinicians in this field to learn and develop.
Main duties of the job
The successful candidate will be responsible for the assessment, and treatment of service users who have a variety of complex needs, diverse presentations, and a range of mental health conditions. The role will include working within the teams physical treatment pathway
In collaboration with the Service User and their families, the post holder will facilitate formulations which will inform Recovery and Wellbeing focussed collaborative Care Plans. Ensuring regular reviews to monitor progress and adapt the Care Plan as necessary.
They will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting resilience, recovery and wellbeing and maximising independence.
