    Job summary

    An exciting opportunity has arisen for a SPLW to join our Socialand Wellbeing Team in Easington Central PCN.

    In your role as a SPLW, you will work as part of theEasington Central PCN Integrated Personalised Care Team, alongside our SPLWs,Care Co-ordinators and Heath Wellbeing coach. This role is for an adult SPLWbut an interest/experience of young person's support is desirable. The post ishosted by one practice but will support patients across the three practiceswithin the PCN.

    Main duties of the job

    Your main role will be to support patients in our localcommunity to take control of their health and wellbeing by:

    Promoting prevention & lifestyle changes

    Engaging patients and connecting them with thewide range of groups and services

    Working with the wider health, social care andvoluntary network

    Encouraging a holistic approach for patientswith chronic diseases, including Cancer Care Coordination.

    Building a relationship in the journey ofpatients towards mental health services

    Bereavement support supporting patientsfamilies and signposting them to an appropriate bereavement service wherenecessary

    Supporting the practices in attaining theirreferral targets

    Promoting and raising awareness of the socialprescribing service within the practices and partner agencies

    Easington Central Primary Care Network is an innovative,dynamic PCN combining 3 local GP Practices based in Easington, with apopulation of over 33,000 patients.

    We are continually seeking new ways to support and improvelocal healthcare services to our patient population.

    We are committed to the ongoing development of our team,which includes mandatory and developmental training for all our staff.

    Social prescribing link workers will work as a key part ofthe Easington Central Primary Care Network (PCN) multidisciplinary team. Socialprescribing can help PCNs to strengthen community and personal resilience,reduce health inequalities (in relation to timely access and outcomes) andwellbeing inequalities by addressing the wider determinants of health, such asdebt, poor housing and physical inactivity, by increasing peoples activeinvolvement with their local communities. It particularly works for people withlong term conditions (including support for mental health), for people who arelonely or isolated, or have complex social needs which affect their wellbeing.

    Job responsibilities

    To work with patients in a variety of settings, in their ownhomes or in GP surgeries, to signpost them to local services/organisationswhich the patient themselves feels would be of benefit to reach their goals.These services may include befriending, weight management, mental healthsupport, smoking cessation, local social groups, leisure centres andvolunteering.

    To work with partner organisations to support thedevelopment and delivery of an integrated care plan for the client so all theirneeds are met in a holistic way.

    To follow up with clients, update their records and supportthem on an ongoing basis.

    To make and maintain contact with local health and wellbeingservice providers and build a network and knowledge of referral routes to andfrom service providers.

    To ensure information about providers and voluntary groupsis shared locally across neighbouring networks.

    To undertake regular training to extend and update knowledgeof health and wellbeing issues.

    Supporting patients with issues around benefits, finances,housing, employment/volunteering, training, education, reducing socialisolation and promoting independence, general wellbeing, mental health support,bereavement, weight management, smoking cessation long term conditions, or anyother non-clinical issues.

    Engaging with patients in a person-centred way,understanding any issues or challenges they may be facing, advising ofdifferent options and choices available to them, linking them up to theappropriate support, by signposting and referring to local organisations toresolve any issues or generally improve their general wellbeing by reducingisolation and promoting independence.

    Working with patients until they are engaging with the rightsupport, offering follow ups, and promoting our SPLW service, reassuringpatients they can return for future support at any time, always empoweringpeople so they have the tools and knowledge to access services for support inthe future.

    Teamwork, flexibility, enthusiasm and theability to prioritise tasks effectively are essential skills.

  • Educated to A level/BTEC or equivalent experience in relevant field

  • Experience in a customer facing role
  • Experience delivering lifestyle change / brief interventions
  • Excellent people skills, interacting with all types of people and able to change communication style to fit the person
  • Ability to work independently and as part of a team
  • Comfortable working with people from public, private and voluntary organisations
  • Highly organised, managing time, resources and competing priorities in a structured and efficient way
  • Highly motivated and willing to go out of your way to help clients
  • Creative problem solver and willing to search for hard to find information
  • Good IT and administrative skills
  • Good understanding of health issues and the wider determinants of health and wellbeing
