Description
As part of the continued expansion of Community Mental Health Services, an exciting opportunity has arisen for the position of Community Psychiatric Nurse at Grays Hall, Thurrock, Grays, Essex.
You will be employed as a Care Co-ordinator, within a Multi-Disciplinary Team, who work with patients with Severe and Enduring Mental Health Illness, including Dual Diagnosis, coupled with history of disengagement, non-compliance with treatment and revolving door admissions.

Our EPUT NHS staff bank is an entity managed by the trust that hires clinical and non-clinical healthcare professionals to take on shifts at our trust hospitals and community settings. Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.
All our permanent staff are automatically enrolled onto the staff bank however this does not mean you have to work any additional shifts, but the option is there for you if you wish.
If you are joining our Trust in a fixed term role, please indicate on your New Starter Paperwork that you wish to join our staff bank. Job description Job responsibilities To act as the named care co-ordinator and manage the coordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers, within a defined catchment area/locality.
To provide professional advice and support to other agencies and members of the multidisciplinary team, and act up for Team Lead in their absence as appropriate. To act as a role model and resource for junior/less experiencedstaff.
The post holder will work under the overall supervision of the Team Leader but is required toundertake the duties of the post without direct supervision.
