    We currently have a number of DistrictNurse vacancies across our District Nursing Networks.The established teams of qualified District Nurses, Senior Nurses, CommunityStaff Nurses, Nursing Associates and Healthcare Support Workers workautonomously to provide high quality nursing care to patients predominantly ina place they call home or a clinic setting.

    This is an exciting time to join our District Nursing service, as wecontinue to develop in response to the changing needs of communities, and arestarting developing our Integrated Neighbourhood Teams.

    Main duties of the job

    Working as part of the District Nursing team you will develop extensive clinical assessment and case management skills. District Nursing teams work closely with their community colleagues to maintain patients independence at home by utilising their excellent networks with our partners and voluntary agencies in local communities, planning and delivering complex packages of care.

    The post holder is responsible formanaging and leading a team to ensure provision of high quality care toclients across the district. Thepost holder will work within a District Nursing team utilising theiradvanced clinical expertise, decision making and problem solving skills toprovide skilled nursing care for patients within their own home or clinicsetting.

    The post holder will beresponsible for managing a caseload of patients, for the assessment of holisticneeds and the implementation of management plans.

    The post holder will have a responsibility to empower team members todevelop within the organisation by demonstrating the principles of leadership.

    Act as an autonomous practitioner, assessingpatients physical, social, psychological, emotional and spiritual needs todevelop a holistic care plan. Working in collaboration with other healthand social care professionals and voluntary agencies to provide a high qualityservice. If legally authorised, prescribe medications from the agreedformularies, based on efficacy, safety and cost.

    Key Responsibilities

  • Lead and clinically direct the professional teamto ensure the implementation and monitoring of standards of care and efficientmanagement of finite resources, supporting and coordinating the teamsworkload. The post holder will have a responsibility to empower team members todevelop within the organisation by demonstrating the principles of leadershipthrough collaboration, assertiveness and risk taking
  • Act as an autonomous practitioner, assessingpatients physical, social, psychological, emotional and spiritual needs todevelop a holistic health care plan, working in collaboration with other healthand social care professionals, and voluntary agencies to provide a high qualityservice.   • Using information from laboratory results agreetreatment options in discussion with the patients, GP and secondary careConsultant as appropriate
  • Initiate andcontribute to strategies designed to promote and improve health and preventdisease working proactively and collaborating with other agencies
  • Maintain record keeping competencies, andpatient confidentiality, in accordance with Mid Yorkshire Hospitals NHS Trustpolicies and the Nursing and Midwifery Code of Conduct. The post holder must beable to structure and write clear records in an accurate contemporaneous manner
  • Demonstrate awareness of wider issues affectingpatients, adult protection, anxiety management, alcohol and drugdependencies, together with strategies and support mechanisms to address these
  • Identify issues pertinent to the patient groupsserved and advocate on their behalf to promote health and well being
  • Discuss with the patient negotiating with healthand social care colleagues, the most appropriate settings in which care can beprovided, accessing services as appropriate and giving consideration to otheralternatives in the patients journey
  • Ensure equipment is obtained, used andmaintained in accordance with Mid Yorkshire Teaching NHS Trust requirements
  • Achieve excellent and effective communication& relationships with all care providers/service users. This requires the post holder to bediplomatic and able to interact with service users/carers in such a way as togain co-operation and avoid conflict. He/she should act as a positive ambassador for the practice andTrust. The post holder must be able todemonstrate a range of interpersonal skills relating to all staff and servicesusers/carers in a confident manner. Thepost holder must also be able to maintain good relationships with colleaguesand work effectively as part of a team
  • Able to communicate verbally by presentinginformation in a coherent manner in both formal and informal arenas
  • Demonstrate diplomacy and be able to deal withstaff and patients in such a way as to avoid conflict
  • Act as a source of advice of local, regional andnational issues relating to the health needs of the population within the MidYorkshire Teaching NHS Trust area demonstrating effective leadership skillswhich expand and develop good practice

  • Undertakeperformance development reviews in line with the KSF to facilitate effectivelearning experiences and achieve clinical outcomes
  • Facilitateeffective learning experiences for students through preceptorship, mentorship,clinical supervision and the provision of an educational environment
  • Facilitate the investigation of complaints,accidents and incidents and assist in the compilation of reports takingrecommended action as supported by line manager
  • Lead andinitiate change within the team within the context of the current politicalclimate, working closely with the modernisation lead
  • Facilitatechange in practice that will improve clinical outcomes and meet the needs ofpatients and care givers that are consistent with national standards andcurrent research
  • Demonstrate awareness and application ofclinical governance with particular attention to clinically effective practice,clinical audit, and risk management. Comply with the Trusts clinical and non-clinical policies andguidelines
  • Responsiblefor recognising ethical and legal issues which have implications for nursingpractice and take the appropriate action.
  • Ensure adherence to the Mid Yorkshire Teaching NHS Trust key performance indicators/targets especially in relation to reducinghospital admission and length of stay. Reporting these as required
