    Job summary

    Hounslow Health Primary Care Network is committed to providing excellent patient centred care for approximately 90,000 residents in Hounslow borough.

    We are looking for an advance clinical practitioner nurse (ACPN) to join our PCN to deliver high quality and effective healthcare to our patients. You will be dynamic, enthusiastic, self-motivated, highly experienced nurse with excellent clinical general practice & knowledge. In this role you will use your skills as an advanced practitioner in diagnostics and therapeutics, enhanced level consultation, critical thinking with clinical decision-making, and leadership.

    Candidate should ideally be practising as an ACPN.

  • Main duties of the job

    You will work as part of a multidisciplinary team to:

  • Work with practices within the PCN to develop effective, evidence-based care for the PCN's population.
  • Contribute to the development of joint standards of care, clinical guidelines and management pathways for utilisation within practices
  • Develop good relationships between local community services, secondary care and the teams within the PCN
  • Be part of the PCN team providing peer support along with education and training

    This employment is under Agenda for Change terms and conditions and is banded as AfC Band 8b. The contract of employment for this role will be hosted by Hounslow and Richmond Community Healthcare trust (HRCH) working on behalf and in partnership with Hounslow GP Consortium whose membership comprises of the 5 Primary Care Networks in Hounslow. The PCN staff will, on a day-to-day basis, be line managed and receive clinical leadership from the PCN. (The contract of employment will be held by Hounslow and Richmond Community Healthcare trust (HRCH). If the successful candidate already works for an NHS employer, then they will get continuity of employment.

    Job responsibilities

  • Treat patients utilising highly developed knowledge and skills, or refer to a range of primary and secondary care services as appropriate.
  • Order and interpret tests and investigations required for diagnosis and management of patients.
  • Review and monitor care plans and therapeutic interventions appropriate to patient need and in accordance with evidence-based practice and national and local guidelines.
  • Prescribe in an independent role
  • Engage with other members of the practice and multidisciplinary team to develop effective, evidence-based care for the practice population.

    Person Specification

    Qualifications

    Essential

  • RGN, Registered first level MSc or equivalent, MSc in Advanced Practice, Independent Prescriber ,evidence of CPD
  • Desirable

  • Mentoring/training qualification
  • Experience

    Essential

  • Recent primary or community nursing experience, Experience in management long term conditions , asthma, COPD, diabetes, CHD, Evidence of working autonomously and as part of a team Proven ability to evaluate the safety and effectiveness of their own clinical practice
  • Desirable

  • Experience in primary care/PCN
  • Skills

    Essential

  • Assess effective patient risk, competent prescriber, manage minor ailments, high level communication skills, IT skills, organised, education&training, conflict management skills, team participation, leadership skills
