    Job Title: District Nurse at Northamptonshire Healthcare NHS Foundation Trust

    Job Summary: District nurses play a crucial role in providing complex care to patients at home, supporting families, and promoting patient care in a familiar environment.

    We are seeking experienced Registered Nurses with a strong commitment to delivering high-quality nursing care, particularly in acute medicine, Emergency Care, or community nursing. The ideal candidate will conduct acute nursing assessments and interventions at patients' homes and facilitate referrals to community services post-care.

    Main Duties of the Job

    Working Life:

    • Assess healthcare needs of patients and families

    Patients may vary in age but often include the elderly, those recently discharged from the hospital, terminally ill individuals, or those with physical disabilities.

    The focus is on delivering top-notch care to enable patients to stay at home comfortably. This role in Northamptonshire involves collaboration within a diverse team to enhance service quality and patient experience, reducing hospital readmissions, and facilitating quick home returns.
