    Description

    Job summary

    Pharmacy Technicians play an important role within GeneralPractice and complement the work of Clinical Pharmacists through utilisation oftheir technical skillset. Their deployment within primary care settings allowsthe application of their acquired pharmaceutical knowledge in tasks such asaudits, discharge management, prescription issuing, and where appropriate,informing patients and other members of the PCN workforce. Work is often underthe direction of Clinical Pharmacists, and this benefit is realised through thecreation of a PCN pharmacy team.

    Main duties of the job

  • To undertake medicines reconciliation in accordance with practice protocols & to update patients medical records/prescribing accordingly.
  • To support the achievement of the practices prescribing Quality & Outcomes Framework targets.
  • To undertake patient-facing consultations, in the context of medication reviews & other clinical services relating to medicines management.
  • Working with the senior pharmacy technician to contribute to the review & development of repeat prescribing protocols/standard operating procedures, to improve the efficiency & effectiveness of repeat prescribing systems.
  • To provide advice to GPs, staff & patients, in changes to prescribing, to support the improvement of prescribing safety, quality & cost effectiveness.
  • To participate in medication audits.

    Job responsibilities

    The post-holder will support the practice based clinical pharmacy team, to ensure the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste & maximising patient outcomes.

    Essential

  • Relevant post-qualification pharmacy experience
  • experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans

    • Desirable

  • be aware of data protection (GDPR) and confidentiality issues particularly within a PCN

    • Qualifications

    Essential

  • Hold a recognised pharmacy technician qualification NVQ level 3 in Pharmacy Services and BTEC level 3 in pharmaceutical Sciences or equivalent
  • Registered with the General Pharmaceutical Council (GPhC)
  • Enrolled in, undertaking or qualified from, an approved training pathway. For example, the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH) or courses approved by the Association of Pharmacy Technicians UK (APTUK) as meeting the requirements of the UK-wide APTUK/PCPA National Competency Framework for Primary Care Pharmacy Technicians.
