    CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting

    CLCH is really proud of a new initiative for qualified nurses to develop a range of clinical skills across a number of nursing specialities with the introduction of a band 5 rotational scheme. This presents an exciting opportunity to work within community services in Hertfordshire.

    Our vision is to provide great care closer to home, involving patients and their carers throughout care delivery. Within our services we support people often living with multiple long term conditions and with numbers expected to grow substantially, it is vital that the workforce is responsive to patient need. We will promote a supportive environment in order for successful candidates to increase their confidence in clinical care delivery, in order to become an autonomous practitioner.

    Main duties of the job

    Our rotation schemes cover 4 x 6 month rotations. During the scheme, applicants may work within Specialist Palliative Care, Respiratory, Planned and Unplanned Care services, or Bladder and Bowel, Tissue Viability/ Lymphoedema, Inpatient rehabilitation and Planned Care services.

    The schemes aim to inspire future career paths available within adult community services. Placements are based across the four localities in the area; Watford and Three Rivers, St Albans and Harpenden, Dacorum and Hertsmere. Many of our services cover 7 days and your placements may include a variety of working patterns.

    Successful applicants will be well supported through a comprehensive induction programme, a buddy scheme and receive mentorship from senior clinicians. You will have access to a variety of training opportunities delivered through CLCH Academy and be part of a wider support network who will support you in gaining a wide range of competencies. There will be a range of different opportunities on offer to develop your skills. At the end of the 2-year scheme, applicants can choose to continue to rotate within the band 5 scheme, or be supported to apply for progression posts, to continue your career journey.

    Job description

    Job responsibilities

  • To give skilled nursing care, based on principles of best practice and clinical evidence. This includes: Chronic disease management, end of life care, continence promotion and wound management.
  • Act as a Named Nurse with the support and supervision of the DN Team Lead. To take continued responsibility for the holistic health needs assessment including care planning, evaluation and appropriate implementation of health care and ensuring continuity of care following referral from acute care, rehabilitation units, GPs and other agencies.
  • Undertake comprehensive assessment of the physical, psychological and social care needs of patients using the Single Assessment Process and including the needs of carers. To provide individualised and person centred care plans that provide monitoring and promotion of self-care to prevent unnecessary hospital admissions

    Qualifications

    Essential

  • Registered Nurse with current UK NMC registration
  • Desirable

  • Evidence of other post-registration education and training
  • Experience

    Essential

  • Recent clinical nursing experience negotiating and liaising within a multi-disciplinary team
  • Working with people with complex health needs
  • Supporting students and other learners HCAs teaching others in the practice environment
  • Experience of undertaking general assessment by utilising evidence-based care.
  • Previous experience in the Nursing process, care planning and fluent in record keeping
  • Experience of providing holistic care that involves the multidisciplinary team.
  • Desirable

  • Experience of training other students.
  • Participation in audit; research projects; service development
  • Post registration experience in a NHS community environment
  • Skills

    Essential

  • Valid UK Driver's Licence and access to a vehicle for work purposes.
  • Knowledge of out of hospital care and the role of the Community Nurse
  • Ability to listen effectively and ability to give constructive feedback to clients and colleagues.
  • Time management skills.
  • Knowledge of how Equal Opportunities can be implemented in practice
  • Knowledge of reasonable response to service user needs and prioritising and dealing with those needs with a required urgency.
  • Awareness of the management and use of data in the NHS setting
  • Knowledge of risks involved in working in the community.
  • Knowledge of evidence based practice.
  • Knowledge of management of long term condition Diabetes and an understanding of the concept of self- management.
  • Knowledge of infection control
