The Senior Social Work Practitioner (SW) provides an autonomous social work service to inpatients on the Neurorehabilitation Unit (NRU) and manages the Social Work Assistant.

The SW has the responsibility for the strategic and operational development of the Social Work service within the NRU in conjunction with the General Manager, working collaboratively with the multi-disciplinary team (MDT) to ensure integrated care for all patients.

This requires ongoing assessment of need and the appropriate intervention and the management of the identified areas of concern.

It also involves setting priorities, initiating service developments, contributing to audit and research, and ensuring that robust systems for clinical governance are in place within and across the Social Work service and multidisciplinary teams.

The SW works with other agencies (e.g. local authorities, benefits agencies, charitable/voluntary organisations such as housing departments, legal and immigration services) to facilitate a seamless transition from hospital to home for patients.

  • To provide an autonomous social work service under the Care Act 2014.To establish the needs of patients with complex neurological disability and those of their carers, and to identify relevant resources to meet them. This involves liaising with various external voluntary and statutory organisations to ensure the development and implementation of a care management plan that provides for a safe discharge for NRU patients.
  • To liaise effectively, with all parties involved in the patient's care locally and nationally, and to maintain appropriate and accurate documentation as an autonomous practitioner.
  • To manage Social Work Assistant and to participate in the recruitment selection and induction of staff when necessary.
  • To act as a resource for all NRU based health care professionals regarding Local Authority policy and practice and advice regarding the impact of any changes in legislation or policy.
  • To promote the implementation of clinical governance and to participate in reviewing clinical practice and policy making.
  • To be committed to implementing practices to promote an integrated multidisciplinary service ensuring holistic client cantered care.

Clinical Responsibilities:


  • To be accountable for the delivery of the Social Work service on the NRU ensuring the delivery of an effective and coordinated admission to discharge service for patients.
  • To ensure that the agreed quality standards of service, based on professional guidelines and competencies are maintained by the SW and the Social Work Assistant.
  • To be professionally and legally accountable and responsible for all aspects of own professional practice.
  • To provide a comprehensive assessment of individual patients in need and their carers, including those in highly complex situations, in conjunction with the MDT.
  • To intervene and provide services to achieve change, through provision or purchase of appropriate levels of support and care.
  • To take the lead role in managing complex patient discharges, coordinating the MDT and external organizations to ensure that the appropriate accommodation, care and financial resources have been arranged.
  • To arrange and chair case review and/or discharge planning meetings with patients, their support network, relevant MDT members and outside agencies as appropriate.
