About the Role:

Phoenix Primary Care Network have an exciting, new opportunity for a Care Co-ordinator to join our Primary Care Network, supporting our Longfield Medical Centre and Tollesbury GP practices.

The role will require meeting with patients, as well as any relevant family members or carers, to discuss what they need and want from their care, and what services are available to them to support their personalised care requirements, signposting and navigation.

They will also work effectively with the clinical team and PCN Social Prescriber to ensure a holistic approach to patient care.

About Us:

Longfield Medical Centre is a seven Partner dispensing and training practice located within the town of Maldon, with a list size of approximately 17,000 patients

Tollesbury Surgery is a two Partner practice located in the rural village of Tollesbury, and have approximately 5,000 patients.

The Phoenix PCN was formed in 2018, and both practices have been working effectively together ever since. The PCN staffing is expanding, and currently employs Clinical Pharmacists, Paramedics and a Social Prescriber.
