    Employer Central London Community Healthcare NHS Trust Location London Salary £42,471 to £50,364 Per Annum, inclusive of HCAS Closing date 14 May 2024 We are looking for an exceptional Band 6 District Nurse Deputy Team Leader to join us in our Central London community nursing service in West London which covers Kensington & Chelsea and North West Westminster.

    The post holder will be expected to undertake the holistic assessment of patients using a person centered approach, and co-ordinate care packages across both health and social care as appropriate to avoid inappropriate admission to hospital or long term care environments, in addition to facilitate the timely discharge of patients from secondary care whilst taking active part in innovative projects that enhance patient centered service delivery and development.

    The post holder will work in collaboration with the designated District Nurse Team Leader in managing the care of patients with long term conditions, incorporating monitoring progress and promoting the preventative aspects of nursing practice.

    To provide skilled, effective, and evidence-based nursing care to patients in a variety of community settings.

    To act as an autonomous Named Nurse taking continued responsibility for first assessments and ongoing assessment, planning, implementing and evaluation of care given to individuals, groups of patients and their families.

    To utilise a case managed approach in the co-ordination of care that aims to prevent and reduce hospital admissions.

    Actively work with the Community Matrons, team leads, GPs and others to identify patients with Long Term Conditions requiring proactive case management.

    Evidence of recent and continuing post-registration professional education and training
    Practice supervisor/assessor training

    Has undertaken formal training in staff and team management
