    Job overview

    The ELFT Continuing Healthcare Team (Newham) has an exciting opportunity for an experienced Registered Nurse on a part time basis (22.5 hrs per week).

    We are looking for a trained nurse with an interest in the specialist area of Continuing Healthcare (CHC). The post holder will be expected to be able to work independently in all community settings. The post holder will be required to become knowledgeable of the National Framework for Continuing Healthcare. There will be opportunities for the post holder to develop their knowledge and skills in the role.

    Clinical supervision will be provided on a monthly basis with a line manager. Also, there are opportunities to discuss cases in the weekly team meeting.

    You will be supported by our team of highly experienced CHC Nurse Specialists to ensure you reach a high standard of clinical knowledge, skills and experience. We will also support the development, implementation and maintenance of high quality, safe, sustainable clinical and operational leadership under the direction of the CHC Team Managers and Clinical Leads.

    We are looking for an individual who will take pride in their work, are highly motivated, well organised, effective in the use of IT, and able to communicate with a wide range of people. They must have a flexible approach in managing their workload and be able to embrace change with balancing the needs of the service. The individual must be able to adapt and work with challenges.

    To be responsible for carrying out NHS Continuing Healthcare (CHC) and NHS Funded Care assessments in a timely manner. The post holder must be able to exercise professional judgement when carrying out CHC assessments.

    You will be required to:

    Use evidence to complete Funded Nursing Care Assessment and Continuing Healthcare Assessments in line with the National Framework for Continuing Healthcare.

    Carry out timely reviews of Funded Nursing Care Assessments, Continuing Healthcare Assessments and Reviews involving families and other agencies.

    The post holder will be expected to manage a generic caseload of clients.

  • The post holder will carry out screening for Continuing Healthcare (CHC) prior to NHS Funded Nursing Care within Care Homes in the London Borough of Newham.
  • The post holder will act as resource for health advice and expertise to Health and Social Care Staff with respect to CHC and NHS Funded Nursing Care.
  • The post holder will have a caseload of patients to case manage.
  • To be responsible for carrying out NHS Continuing Healthcare (CHC) and NHS Funded Care assessments in a timely manner. The post holder must be able to exercise professional judgement when carrying out CHC assessments. These assessments need to be completed within the National Framework Guidelines of 28 days.
  • To keep North East London Integrated Care Board informed of any relevant changes which may affect CHC/NHS Funded payments.
  • To maximise patients' potential for independent living at home; through welfare home visits, appropriate referrals and care interventions.
  • To be sensitive to the health and cultural needs specific to the ethnic minority population of Newham, using the health service advocacy where appropriate.
  • To build up a professional working relationship and trust with the healthcare professional team working with nursing home patients, and in the community by visiting homes on a regular basis, participating in local meetings and providing feedback.

  • Registered Adult Nurse on Part 1 of the NMC register.
  • Evidence of CPD.

  • Experience and knowledge of safeguarding concerns and working in conjunction with appropriate agencies
