    Job summary

    An exciting opportunityhas arisen for an enthusiastic, well-motivated and flexible Deputy ClinicalTeam Manager to join our Urgent Community Response andVirtual Ward Services across Sunderland.

    This role is based with the Recovery at Hometeam based at Leechmere Independent Living Centre in Sunderland.

    The Recovery at Home Team consistsof an extensive MDT which includes nurse practitioners, nurses, occupationaltherapists, physiotherapists, health care assistants, therapyassistant/exercise practitioners and administrative staff. The team also worksclosely alongside partner agencies including those from the local and voluntarysector.

    Main duties of the job

    The SunderlandHome Support service is based within Recovery at Home and consists of Band 2Health Care Assistants who deliver short term care support to patients in theirown home with the aim of facilitating discharges from hospital and preventingavoidable hospital admissions.

    This role would involve leading the service of Sunderland HomeSupport within the Recovery at Home Team with the support of the Clinical TeamManagers.

    The role involves line management of the SunderlandHome Support team, with overall accountability and responsibility for thedelivery of all aspects of patient / client care through the provision ofeffective clinical and managerial leadership

    The service is fast pacedand you would be expected to see a wide range of patients who present with avariety of conditions.

    This is a unique andrewarding role which will allow you to work with patients within their usualplace of residence to enable them to remain there safely alongside youdeveloping your own clinical and leadership skills.

    The service is committed to providing a soundenvironment for education and training to support the successful candidatewithin this role.

    Attendance and contribution at team meetings

    Workautonomously as a nurse practitioner using advanced and enhanced skills,assessing individual patient needs, initiating investigations, determining adifferential diagnosis and initiating appropriate holistic, research basedtreatment and care.

    Actas a patient advocate in all matters relating to their care.

    Support more junior staff to develop the necessary knowledge, skill andcompetence to provide high quality services.

    Person Specification

    Qualifications

    Essential

  • Current NMC Registration (Level 1 adult nurse).
  • Degree in nursing studies or healthcare related subject or evidence of currently working towards this (UK equivalent).
  • Mentorship certificate/ Practice Assessor training or demonstrable equivalent experience.
  • Desirable

  • Management and leadership training
  • ILM qualification.
  • Community Experience
  • Clinical skills Qualification (willing to undertake)
  • Experience

    Essential

  • Experience as a Band 5 nurse
  • Experience of leading a team.
  • Evidence of innovation in practice.
  • Experience of coordinating a team in caring for patients from admission to discharge.
  • Desirable

  • Expertise within the speciality.
  • Experience of line management of staff
  • Skills and Knowledge

    Essential

  • Awareness of national health and social care policy.
  • Knowledge of best practice in area of specialism.
  • Thorough understanding of expanded roles/professional practice issues.
  • Clinical teaching experience.
  • Ability to provide and receive complex, sensitive or contentious information, where persuasive, motivational, negotiating, training, empathic or reassurance skills are required.
  • Able to work without supervision.
  • Competent in the use of IT packages.
  • Proven interpersonal skills.
  • Evidence of competency in relation to nursing practices appropriate to the post.

  • Understanding of budget and financial control systems.

  • Ability to assist patients to move as per Trust Moving and Handling Policy
  • Car owner with full UK licence
