Description

We are looking for an enthusiastic, dynamic, flexible and highly motivated nurse to join a team providing person centred care across several small residential establishments in Cardiff and The Vale of Glamorgan.


We provide a support led service to individuals of age 12 and over with complex mental health needs and / or learning disabilities across several small residential care homes in Cardiff and Barry.

We promote a "can do" approach that seeks to enable individuals to meet their full potential and gives them a strong voice with regards to decision making and lifestyle choices.


Experience in the social care sector, experience of dealing with behaviours of concern and positive behaviour support and management is essential.


You will be expected to contribute towards creating a best practice environment as an integral part of a dedicated team, where everyone is valued, and professional development encouraged.

The role will include some training and mentoring of our care staff and auditing of clinical compliance and competencies.

You will have direct input into development of company policies and procedures and responsibility for ensuring that the clinical needs of the individuals within our homes are met.


MAIN OBJECTIVES OF THE POST -

  • Play a key role in ensuring the physical, mental and emotional wellbeing of individuals is monitored and maintained.
  • Oversee the assessment, development, implementation and review of programs aimed at enabling individuals to manage any behaviours of concern, in line with their PBS plan.
  • Undertake behavioural reviews, identify triggers and patterns and proactive responses to lessen behaviours.
  • Provide leadership, training and mentoring for staff to ensure the development of best care practice and optimal outcomes for the individuals we support.
  • Monitor the competencies of staff following appropriate training and follow up as necessary.
  • Support care home managers in the assessment, development, implementation and review of individual care needs.
  • Keep up to date with new developments and legislation within the health and social care sector and contribute to the writing and review of policy and procedure, helping to ensure they are implemented across the organisation.
  • Undertake clinical reviews and risk assessments and audit care plans alongside managers and team leaders, ensuring they meet the individual's needs.
  • Work in partnership with families, friends, advocates and members of professional networks to promote the health, welfare and development of individuals; liaise with external agencies and multi disciplinary teams advocating in the individual's best interest.
  • Carry out an element of out of hours duties
  • Play a part in the referral process, assessing individuals who are looking for potential placements
  • Deputise if necessary in the absence any of our care managers and take lead responsibility for oversight of any care home including being the nominated person in cases of emergency. Support individuals in a way which promotes their independence, equality, diversity and rights and respects the dignity of the individual, ensuring confidentiality and their safety at all time.
  • Participate in internal audits in line with RISCA regulations.

£36,000.00-£40,000.00 per year
