    Job overview

    An opportunity has arisen for a 11 month development secondment (with the possibility of extension) as District Nurse Team lead for the South Locality, Tower Hamlets Community Health Services.

    The post holder will be a motivated, experienced and highly skilled Community Nurse with excellent communication skills and with the District Nursing Qualification.

    They will lead and support the Locality team to deliver care to patients admitted to the Community Health Team caseload.

    They will work closely with the Locality Clinical Lead to ensure that care provided is of a high quality and meets the required standard.

    They will also champion and facilitate integrated working across the Locality, with key partners in Primary Care, Social Care, Home Care and Mental Heath services.

    They will be offered ongoing support including regular supervision, leadership and management training, and opportunities for practice development.

    Main duties of the job

  • Will ensure that the case management model of delivery: including the effective assessment, planning, implementation, evaluation of care and the discharge of patients from the case load meets the professional and clinical standards/guidelines at all times.
  • Will have line management responsibility for the human resources of the nursing team, the services provided by the team and will be professionally accountable for the appropriate delegation of patient care to nurses within the team and for the supervision of their work and will monitor expenditure within the team.
  • Lead, manage, monitor, prioritise and analyse complex and unpredictable caseloads and lead and manage teams of staff who work autonomously, often in isolation including supporting learners within the team.
  • Role model the values of high quality, compassionate nursing and support the ongoing development of these in others, whilst demonstrating resilience and autonomy in the context of increasing demand, managing change to meet the evolving shape of services through flexibility, innovation and strategic leadership.
  • The District Nurse Team Leader will ensure that a high quality service is provided in patients homes or residential care settings within available resources.
  • Will be responsible for effectively managing a team of support staff and other Trust resources. He/she will be responsible within his/her team for liaison and effective working arrangements with General Practitioners and other agencies

  • RGN
  • DN Cert./Dip/Degree
  • Independent Nurse Prescriber or working towards
  • Evidence of continuing professional education and training at degree level
  • Experience

  • Experience of co-coordinating and managing a team.
  • Ability to liaise effectively with members of the Primary Health Care Team and other agencies
  • Ability to organize and deliver a high standard of evidence research based clinical nursing care
  • Ability to assess, monitor, plan and evaluate patient care.

    Essential criteria

  • Understanding and knowledge of the complex needs of individuals and families in a deprived, urban area
