- Will ensure that the case management model of delivery: including the effective assessment, planning, implementation, evaluation of care and the discharge of patients from the case load meets the professional and clinical standards/guidelines at all times.
- Will have line management responsibility for the human resources of the nursing team, the services provided by the team and will be professionally accountable for the appropriate delegation of patient care to nurses within the team and for the supervision of their work and will monitor expenditure within the team.

Job overview
An opportunity has arisen for a 11 month development secondment (with the possibility of extension) as District Nurse Team lead for the South Locality, Tower Hamlets Community Health Services.
The post holder will be a motivated, experienced and highly skilled Community Nurse with excellent communication skills and with the District Nursing Qualification.
They will lead and support the Locality team to deliver care to patients admitted to the Community Health Team caseload.
They will work closely with the Locality Clinical Lead to ensure that care provided is of a high quality and meets the required standard.
They will also champion and facilitate integrated working across the Locality, with key partners in Primary Care, Social Care, Home Care and Mental Heath services.
They will be offered ongoing support including regular supervision, leadership and management training, and opportunities for practice development.
