    West Leicestershire CMHT are offering an exciting opportunity for a Band 6 Community Mental Health Nurse. The successful candidate will be enthusiastic, positive, and willing to learn. We would be happy to consider applications from experienced Band 5 Mental Health nurses.

    The team is friendly, dynamic and constantly evolving to offer the best support and treatment to patients within our local community. Work alongside a variety of Professionals within the Team and external agencies including GPs, Mental Health Facilitators, Mental Health Practitioners, Social Services and theVoluntary Sector, to obtain the best recovery possible for our patients.

    Main duties of the job

    As a CPN you will work collaboratively with the multi-disciplinary team (MDT) to achieve best mental health recovery for our patients.

    You will be responsible for a caseload of patients of your own to support, guide and ultimately enable independence of the patients.

    CPN's also conduct thorough assessments and complete clinical triages to support the team manage incoming referrals.

    CPN's participate in the Duty Rota where you act as single point of contact for patient enquiries on the day.
