- To work as a practitioner delivering nursing care within Peritoneal Dialysis to a defined caseload of patients / clients.
- To work as an effective lead within the Peritoneal Dialysis (PD) Service.
- To provide evidence based care in line with current research and guidelines, offering advice and clinical support to others regarding all aspects of PD.
- To maintain clear lines of communication with primary healthcare teams in order to facilitate the care of this group of people in the community.
- To be an active member of the multidisciplinary team.
- To provide specialist nursing advice and support to patients, their families and other healthcare professionals following diagnosis and through treatment with the ability to demonstrate clinical expertise in the care of patients requiring renal replacement therapy.
- To ensure continuity of a high standard of evidence based nursing care, assessing health, health related and nursing needs of patients, their families and other carers.
- To act as a resource person to others, providing accurate information, advice and support to the MDT and patients/ relatives both within the hospital and community setting.
- To manage own patient caseload under supervision.
- To be actively involved with in-patient care, liaising with ward staff to ensure high quality nursing care.
- To attend all relevant Multidisciplinary Team (MDT) meetings and actively contribute.
- To follow-up patients under supervision as appropriate and ensure all relevant information is available to enable patients to make an informed choice about their treatment.
- To liaise with the ward staff to ensure appropriate and timely discharge / follow-up arrangements are in place for patients.
- To liaise with local community, social services, paramedics and voluntary organisations regarding current service availability.
- To participate in the development of appropriate patient group directives to facilitate best practice.
- To be responsible for running appropriate Nurse-led clinics.
