    An exciting opportunity has arisen to join The Right Care Hub team.

    We are currently seeking seeking Care Coordinators to join our fantastic dedicated team within The Right Care Hub. You will need to demonstrate excellent interpersonal and communication skills together with a good understanding of community services.

    The successful candidate will demonstrate that they are willing to support innovation, and are committed to service improvement and delivery in an extremely busy and challenging environment.

    Main duties of the job We are looking for an enthusiastic and forward thinking care coordinator to join our busy, pro-active hub.

    Based within The Right Care Hub at St Catherine's Health Centre in Birkenhead, your main duties will include working primarily with Wirral GPs and other multi-disciplinary professionals via our dedicated GP Single Point of Access.

    The successful post holder will support in the signposting of other professionals using a directory of services. You will guide GP's to the most appropriate referral pathway for the patient ensuring that the patient journey is safe and suitable and avoid unnecessary hospital admissions.

    You need to have significant NHS experience, preferably within a community setting, have a good understanding of community services, risk management and working as part of a multi-disciplinary team. Excellent communication skills and an ability to deal with repetitive calls at pace are essential. You will need good time management skills.

    You will manage a team facilitating supervisions and all HR processes. There will be the opportunity for supervision and to attend in-service training and professional development. Job description Job responsibilities You will use established pathways and processes to provide a wide range of options to support Primary care and other system partners in determining the most appropriate service for patients who may potentially be at risk of a hospital admission. This is a clinical service so having knowledge of conditions is an advantage. The post holder will be under the supervision of a senior nurse practitioner and will escalate in line with trust policies. Person Specification Qualifications and professional training Essential
    • NVQ Level 3 in care management or equivalent health and social care discipline.
    • Evidence of Continuing Professional Development (CPD).
    Desirable
    • ECDL or equivalent ICT qualification.
    Experience and knowledge Essential
    • Broad practical and theoretical understanding of a particular service area generally based on a combination of formal academic study and/or substantial relevant practical management experience
    • 3 years Proven experience of working in a Health or Social care environment role
    • Experienced supervisor, with evidence of managing a team during busy periods.
    • Evidence of understanding / complexity of issues locally and nationally pertaining to management of adult with complex health conditions.
    • Experience of working independently, unsupervised providing high quality care.
    • Evidence of working in multi-disciplinary services.
    • Knowledge of clinical governance agenda,
    • Demonstrate substantive knowledge and skills gained working within a Health & Social Care setting.
    • Demonstrate excellent interpersonal skills and communication skills and the ability to interact and engage with key partners across health and social care and associated businesses.
    • Demonstrate operational experience of managing complex workloads and organisational situations.
    • Demonstrate experience of successful multi-agency working and evidence and outcome based approach.
    • Demonstrate experience and oversight of care coordination and processes.
    • Knowledge of acute and community NHS care pathways
    • Knowledge and experience of maintaining robust data capture systems and processes and monitoring in relation to Key Performance Indicators.
    • Understanding of the challenges confronting health and social care economies and the reality of changing demography and fewer resources to respond to growing population needs.
    • Understanding of the needs of older people / adults with disabilities/long term conditions/ health problems particularly in relation to promoting their independence.
    • Basic knowledge of long term conditions and the complexities involved, both social and medical and the understanding of when specialist medical intervention is needed.
    • Knowledge of complex discharge planning and alternatives to admission.
    • Knowledge of equality and diversity and how it affects staff and people who use services.
    • Ability to operate and lead in a within the clinical governance framework
    • Experience of working in operational and care management services.

    • Ability to analyse and interpret data working with the team to produce required reports.

    • Ability to work with other professionals/service users/carers/families and promote partnership working.
    • Previous experience of maintaining records within a patient management system
    • Ability to contribute to service development
    • Computer literate.
    • Ability to work under pressure and respond quickly to unpredictable caseloads
    • Effective self-organisational and prioritisation skills.
    • An ability to work effectively with stakeholders.
    • Ability to negotiate and influence across organisational boundaries to deliver person centred care.

    • Business planning and administrative skills.
    Personal qualities Essential
    • Expected to work flexibly and be able to work an extended day and/or over a seven day period, 52 weeks of the year if required by the service.
    • Able to travel around the Wirral using private or public transport.
