    Job Title: Band 5 Staff Practitioner

    Job Summary:

    • We are seeking a registered LD/MH Nurse, Occupational Therapist, or Speech & Language Therapist to join our Intensive Support Team (IST) based in Chesterfield.
    • Focus on improving the lives of individuals with learning disabilities.
    • Collaborate with multi-disciplinary LD Community Services, care providers, social care, and inpatient units.

    Main Duties:

    • Play a crucial role in the multi-disciplinary team.
    • Utilize your expertise in behavior, mental health issues, and conditions like Autism.
    • Engage with service users, families, and other stakeholders.
    • Demonstrate exceptional communication, flexibility, and the ability to deliver high-quality care.

    Person Specification:

    Qualifications

    Essential

    • Registered RNLD / RMN / OT / SLT
    • Demonstrated willingness to pursue further training

    Desirable

    • Mentor / Supervisor Qualification
    • Recent and relevant post-registration experience

    Experience and Knowledge

    Essential

    • Evidence of current relevant legislation (MHA, MCA, DoLS)
    • Awareness of Safeguarding Vulnerable Adults
    • Understanding of key service drivers like Transforming Care and Building the Right Support

    Desirable

    • Familiarity with relevant processes (CPA, Person-Centered Care)
    • Experience in supervising others
    • Knowledge of broader NHS initiatives (STP, CHC, Public Health, Health Promotion)
