    Job summary

    An opportunity has arisen for a full time(fixed term contract 12 months) Community Substance Misuse Practitioner to jointhe Specialist outreach recovery team.

    We are looking for an experienced practitioner(RMN, OT or social worker) within the substance misuse field to manage acaseload of clients with complex needs including related mental health andphysical health concerns within the Personalised care framework.

    The role also involves working with othermembers of the MDT, and working within an overall recovery focused treatmentmodel. The team will work closely with other specialist third sector andstatutory substance misuse agencies such as complex needs and Harbour services.

    The role will involve a specialistconsultation, support and involvement with the MDT in care planning and riskmanagement. The successful applicant might joint work with other colleagueswhen there is expertise required in the substance misuse field. The role willalso include an assessment function where there is a need to ascertain needsand facilitate care planning. Experience in adult mental health and/or generalhealth care settings would also be an advantage.

    Main duties of the job

    The postholder is required to act as key worker within the PCF framework for a reduced clinicalcaseload of service users with more complex needs, co-occurring mentalhealth problems, substance misuse and physical health complications.

    This willinclude:

    Comprehensiveassessment of the service users substance misuse problem including theirhealth and wellbeing needs and level of motivation to change.

    agreeing a care plan/aftercare / discharge plan with the service user, which may include otherprofessionals or onward referral as appropriate.

    Implementation andreview of the care plan and associated evaluation in line with the service.

    specific interventions / protective measures(child protection, domestic violence, protection of vulnerable adults etc)

    Respond to, record and report any adverseevent or incidents using the incident report systems within Livewell.

    WORKING AS PART OF A TEAM

    TheCommunity Substance Misuse Nurse will support the Team Manager & Practicelead in overseeing the effective delivery of the locality service and inproviding clinical leadership and supervision for the Team, which may includeleading clinical meetings or other tasks as directed by the Team Manager orPractice lead.

    The postholder will hold line management responsibilities within the team, have aresponsibility of completing line management supervision, annual appraisals andsupporting staff in developing a personal development plan.

    Job responsibilities

    JOB SUMMARY / PURPOSE

    TheCommunity Substance Misuse Practitioner will take responsibility for the keyworking or work alongside people who are key worked elsewhere within the teamof a small clinical caseload of clients with substance misuse, supporting andincluding those with more complex health needs including co-occurring mentalhealth / substance misuse and physical health complications.

    To supportthe Team Manager & Practice Lead in providing clinical leadership andsupervision particularly in regard to complex health needs

    To workin conjunction with the Team Manager and take the lead on agreed areas in theoverall development, monitoring and evaluation of the service.

    To workin close collaboration with other partner agencies and act as a source ofspecialist advice in regard to individual service user care and servicedelivery.

    Toprovide specialist guidance, consultations and support to the team with careplanning and risk management.

    MAIN DUTIES AND RESPONSIBILITIES

    Assessment & CarePlanning & Risk assessment

    The postholder will be expected to carry out comprehensive assessments of risks toservice users and others health and wellbeing. As part of their clinicalduties the post holder will prepare risk assessment tools and management thatare based on thorough risk assessment and will provide protective interventionsto help manage risk.

    review of the risk assessment and crisis managementplan at regular intervals and as circumstances change.

    The postholder will have an understanding of the relevant legislative frameworks and willparticipate in relevant meetings related to the management of risk including Adultsafeguarding, Child Protection meetings and Criminal Justice Risk ManagementForums.

    Identifyand support with a holistic care plan will, that identify goals and specificactivities and interventions to be undertaken within the context of theiroverall care and their health and wellbeing needs.

    Enablingclients to address their emotional, mental, physical, social and spiritualhealth and wellbeing needs.

    The postholder is expected to respect peoples dignity, wishes and beliefs, involvethem in shared decision-making and obtain their consent to share informationwith identified individuals / agencies on a need-to-know basis.

    Interventions& treatment intervention are generally provided within a framework of PCF.

    This postholder will be responsible to deliver clinical supervision,consultations/specialist advice, case review, case management meeting, andcomplex client review.

    The postholder will care coordinate a reduced clinical caseload of service users likelywith a dual diagnosis or those with significant substance misuse difficultiescovering the citywide location.

    The postholder will plan, deliver and evaluate interventions and/or treatments whenthere are complex issues and/or serious mental illness

    Thesuccessful applicant will share the treatment plan of individuals routinelywith the MDT

    Alert theline manager when there are unexpected changes in individuals health andwellbeing or level of risk, discuss and plan with the MDT and any other wideragencies

    Respond to, record and report any adverseevent or incidents using the incident report systems within Livewell

    The postholder may be asked to contribute to the recruitment and selection of teammembers to meet organisational needs consistent with legislation, policies andprocedures.

    The postholder will need to be dynamic, flexible and contribute to a positive teamculture and ethos.

    The postholder will need to work within their competencies.

    COMMUNICATION

    The CommunitySubstance Misuse Practitioner is required to develop and maintain communication

    Tosupport distressed staff. Support will usually be accessible through linemanagement or peers, face to face or over the telephone.

    FINANCAL RESPONSIBILITY

    The postholder does not have financial responsibility for budgetary management.However, it is the individual post holders responsibility to manageorganisational resources efficiently and to suggest efficiency savings to theTeam Manager as they arise. The post holder has the responsibility to maintainthe safekeeping of the organisations property and safeguard the property ofpatients and visitors.

    ADDITIONAL INFORMATION FOR ALLPOSTS

    The postholder is required to comply with all relevant policies and procedurespertinent to their post. Current versions can be found on the intranet or viayour manager.     Risk Management

    In accordance with the Risk Management Strategy, employees willparticipate, whenever required, with the risk management process. They willsupport line managers by attending mandatory and statutory training, completingincident/accident forms for every adverse event or near miss that occurs,report all defects and complaints, and communicate any dangerous situation toindividuals potentially at risk.

    Health and Safety at Work

    You must co-operate with those in authority and others in meeting thestatutory requirements and in following policies and procedures. A copy of the Health and Safety Policy isavailable from the intranet or from the Risk Management Department.

    You are reminded that in accordance with the Health and Safety at WorkAct 1974 you have a duty to take reasonable care to avoid injury to yourselfand to others affected by your work activities.

    You will be notified where your post carries a requirement forimmunisation.

    You may berequired to be able to undertake physical intervention training and participatein physical intervention as part of a physical intervention team and Basic LifeSupport (BLS).

    I nfection Control

    LivewellSouthwest is determined to eradicate healthcare-acquired infection and puts agreat deal of emphasis on the responsibility of all staff to ensure their ownpersonal and others compliance with Infection Control (including Hand Washing)Policies. All staffmust comply with infection control policies and guidance, attend relevantupdates and report issues of concern to their immediate line manager (if noaction or explanation received, then it is the individuals responsibility toescalate their concerns to the Director of Operations or Chief ExecutivesOffice).

    Safeguarding Children andAdults

    All employees have a duty to safeguard and promote the welfare ofchildren and adults and are required to act in such a way that at all timessafeguards their health and wellbeing. Familiarisation with and adherenceto national and local safeguarding adults and children policies is an essentialrequirement upon all employees. Livewell Southwest has specific safeguardingpolicies and in addition, employees also have a responsibility to practice andwork within the multi-agency policy developed by the Safeguarding Adults Boardand the Safeguarding Children Board. Staff are also required to participate inrelated mandatory/statutory training.

    For clinicalposts at bands 5 and 6, there is an expectation that engagement in researchwill be part of this role. Therefore, an awareness of the value and relevanceof research is expected and the post holder should either support existingresearch within their specialty area as appropriate or identify opportunitiesto raise awareness of research possibilities.For clinical posts at band 7 and above, the post holder will be requiredto actively participate in complex audits using research methodology, orparticipate as required in clinical trials or equipment testing, and willdemonstrate high level involvement in local ongoing research projects.
