This post is for a qualified ACP with a passion for continuing the care and treatment for patients with a diagnosis of stroke and other conditions.
To work with the PCN Lead ACP and wider team to continually improve the quality of care provided including the continuation of service improvement to demonstrate better outcomes for our patients.
Your Health Partnership (YHP) is a cutting-edge Primary Care Network working as part of Sandwell and West Birmingham Hospitals Trust.
Sandwell and West Birmingham NHS Trust (SWBT) is an integrated care organisation with a budget of approximately 600 million and over 7,000 staff.
Diversity and social care is at the core of what we do as the Trust provides Community and Acute Services to over half a million people in an urban centre that demands massive regeneration and has substantial premature mortality.
Our new acute hospital, the Midland Metropolitan University Hospital (MMUH), opens in 2024 and will provide care to our local population from first class, purpose-built premises.
The development of the new hospital will play an important role in the regeneration of the wider area and in improving the lives of local people and reducing health inequalities.
Master's level education in relevant health related subjectEvidence of continuous professional development and the practical application of new skills to the workplace
Direct supervision of junior staff
Interest and Knowledge of current Stroke Policies, care and treatment pathways
Experience of multiskilled approach to Chronic Disease Management
Experience of working within an ACP post within primary or community care

Ability to deal with complaints sensitively, avoiding escalation where possible
