    We have an appealing opportunity for a registered Nurse to join our service as a band 6 and work alongside our multidisciplinary team.

    We are looking for an enthusiastic, motivated, and friendly individual who shares the same values as the team, that being putting service users first and in the centre of their care.

    We work with services users aged 18-65, who have moderate to severe mental health issues. Our base is a community team hub, where we work alongside other community services. This helps to provide flexibility, openness and ease for support and advice. Its good working with different disciplines and having learning opportunities from other professions.

    Applications for nurses who are a band 5 will be considered and there is potential for this position to be a development post.

    They will be responsible for arranging care, liaison with other professionals and agencies as well as managing and reviewing care plans and risk assessments, ensuring these are done collaboratively and in a recovery focused manner.

    The successful candidate will be confident in the administration of depot medication and be able to work alongside other Nurses in supporting with the depot clinic as and when required.

    They will take responsibility as the duty worker on a rota basis, providing support to the wider core team caseload which may include medication queries, concerns from family members and generalised support.

    This may also include responding to crisis calls from the wider caseload when on the duty rota.
    The successful candidate should be able to support with and offer supervision to junior staff within the team.

    The successful candidate will be an efficient organiser, managing their workload and be flexible to meet the needs of service.

    Assesses needs and identifies problems relevant to the care of people referred.

    ~ Devises a plan of care in partnership with the person and carers.

    ~ Implements the planned programme of care to ensure a high standard is achieved.

    ~ Reviews the effectiveness of the care provided and where appropriate initiates any action.

    ~ Advises and supports the person and carers to promote health and well-being and to prevent illness.

    ~ Recognises situations that may be detrimental to the health and well-being of the person and initiates action that may be required.

    ~ Establishes and maintains satisfactory communication with all agencies involved in the persons care in both hospital and community.

    ~ Operational Policy and participates in the team duty rota.

    ~ Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.

    ~ Maintains accurate patient care records in accordance with professional codes of conduct and trust procedures and ensures records are made available when required by the appropriate agency.
    Participates in the formulation of policies at unit level.
    Supervises students and trainees on placement.

    ~ Assists in the teaching and in-service training of staff.

    ~ Attends courses or training sessions relevant to the updating of knowledge and experience.

    Promotes an understanding of mental health and well being.

    ~ Ensures health and safety regulations are observed.

    ~ Attends mandatory training sessions as directed by the Trust.

    ~ Supports the Trusts commitment to a healthy work-life balance.

    RMN Current NMC registration.
    Significant experience of working within a mental health setting.
    Can demonstrate skills in assessing and managing people experiencing mental health problems.
    Experience of supporting/mentoring and supervising colleagues/students/trainees.
    Can demonstrate skills in assessing and managing risk in a variety of settings.
    Ability to promote and develop links with other professionals e.g. practitioners within long term conditions, primary care.
    NICE, CQC, CQUINs.

    Experience of working holistically with Adults with an emphasis on coexisting mental and physical health needs that impact on wellbeing.

    Training
    Assessment and care planning skills.
    Risk assessment and risk management.
    Relevant post-registration qualifications and / or training.

    Mental Health Act, Health & Safety, Care Programme Approach, Care Act and Safeguarding.

    Can evidence awareness of equality and diversity and is able to incorporate this into care planning and the delivery of care.

    Knowledge and skills in specialist areas such as dual diagnosis, peri-natal mental health, working with people experiencing psychosis, working with people with a diagnosis of personality disorder


    A satisfactory sickness record over the previous 2 years (subject to the need to act with fairness and equality of opportunity, particularly where the sickness is related to a disability and/or pregnancy).

    Ability to travel across the Trust as appropriate to the role.
