    Job summary

    This is an exciting opportunity to join SouthLincoln Healthcare Federation as part of our expanding MDT.

    The role is patient facing and will involveclinically assessing and treating patients using your expert knowledge ofmedicines for specific disease areas, provide expertise in medication reviews,support patients to manage LTCs through medicines optimisation and be involvedin our innovative care home service.

    The post holder will work with our existingClinical Pharmacists (CP), across our practices (Bassingham Surgery, Branston& Heighington Family Practice, Brant Road & Springcliffe Surgery,Church Walk Surgery, Cliff Villages Medical Practice, The Heath Surgery,Washingborough Family Practice).

    The post holder will be supported to develop therole with the CPPE diploma and non-medical prescribing, if they do not alreadyhold these qualifications.

    Job description

    Job responsibilities

    The wide scope of this role is not an expectationof where the post holder will start but where they aim to be as they achievethe CPPE diploma, non-medical prescribing qualification and gain associatedexperience.

    Patient facing long-term condition clinics

    See (where appropriate) patients withsingle or multiple medical problems where medicine optimisation is required( COPD, asthma).

    Review the on-going need for eachmedicine, a review of monitoring needs and an opportunity to support patientswith their medicines taking ensuring they get the best use of their medicines( medicines optimisation). Make appropriate recommendations to SeniorPharmacists or GPs for medicine improvement.

    Patient facing clinical medication review

    Undertake clinical medication reviewswith patients and produce recommendations for senior clinical pharmacist,nurses and/or GP on prescribing and monitoring.

    Patient facing care home medication reviews

    Undertake clinical medication reviewswith patients and produce recommendations for the senior clinical pharmacist,nurses or GPs on prescribing and monitoring. Work with care home staff toimprove safety of medicines ordering and administration.

    Patient facing domiciliary clinical medicationreview

    Undertake clinical medication reviewswith patients and produce recommendations for the senior clinical pharmacists,nurses and GPs on prescribing and monitoring. Attend and refer patients tomultidisciplinary case conferences.

    Management of common/minor/self-limiting ailments

    Manage caseload of patients withcommon/minor/self-limiting ailments while working within a scope of practiceand limits of competence.

    Signpost to community pharmacy andrefer to GPs or other healthcare professionals where appropriate.

    Patient facing medicines support

    Provide patient facing clinics forthose with medicines queries.

    Telephone medicines support

    Provide a telephone help line forpatients with questions, queries and concerns about their medicines.

    Medicine information to practice staff and patients

    Answer relevant medicine-relatedenquiries from GPs, other practice staff, other healthcare teams (community pharmacy) and patients with queries about medicines. Suggest andrecommend solutions.

    Provide follow up for patients tomonitor the effect of any changes.

    Unplanned hospital admissions

    Review the use of medicines mostcommonly associated with unplanned hospital admissions and readmissions throughaudit and individual patient reviews.

    Put in place changes to reduce theprescribing of these medicines to high risk patient groups.

    Management of medicines at discharge from hospital

    Reconcile medicines followingdischarge from hospitals, intermediate care and into care homes, includingidentifying and rectifying unexplained changes and working with patients andcommunity pharmacists to ensure patients receive the medicines they need postdischarge. Set up and manage systems to ensure continuity of medicines supplyto high risk groups of patients.

    Signposting

    Ensure patients are referred to theappropriate healthcare professional for the appropriate level of care within anappropriate period of time.

    Repeat prescribing

    Produce and implement a practicerepeat prescribing policy.

    Manage the repeat prescribingreauthorisation process by reviewing patient requests for repeat prescriptionsand reviewing medicines reaching review dates and flagging up those needing areview. Ensure patients have appropriate monitoring tests in place whenrequired.

    Risk stratification

    Identify cohorts of patients at highrisk of harm from medicines through pre-prepared practice computer searches.This might include risks that are patient related, medicine related, or both.

    Service development

    Contribute pharmaceutical advice forthe development and implementation of new services that have medicinalcomponents.

    Information management

    Analyse, interpret and presentmedicines data to highlight issues and risks to support decision- making.

    Medicines quality improvement

    Undertake clinical audits ofprescribing in areas directed by the GPs, feedback results and implementchanges in conjunction with the practice team.

    Medicines safety

    Implement changes to medicines thatresult from MHRA alerts, product withdrawal and other local and nationalguidance.

    Implementation of local and national guidelines andformulary recommendations

    Monitor practice prescribing againstthe local health economy's RAG list and make recommendations to GPs formedicines that should be prescribed by hospital doctors (red drugs) or subjectto shared care (amber drugs).

    Assist practices in seeing andmaintaining a practice formulary that is hosted on the practices computersystem. Auditing practices compliance against NICE technology assessmentguidance. Provide newsletters or bulletins on important prescribing messages.

    Education and Training

    Provide education and training toprimary healthcare team on therapeutics and medicines optimisation.

    The post holder will:

    contribute to a patient safetyculture through reporting and investigation of incidents and undertakingproactive measures to improve patient safety

    actively signpost patients to thecorrect healthcare professional

    maintain accurate clinical records ofall patient consultations and related work

    review the latest guidance ensuringthe practice conforms to NICE, CQC etc.

    support in the delivery of enhancedservices and other service requirements on behalf of the PCN

    participate in the management ofpatient complaints when requested to do so and participate in theidentification of any necessary learning brought about through clinicalincidents and near-miss events

    undertake all mandatory training andinduction programmes

    contribute to and embrace thespectrum of clinical governance

    attend a formal appraisal with theirmanager at least every 12 months. Once a performance/training objective hasbeen set, progress will be reviewed on a regular basis so that new objectivescan be agreed

    contribute to public health campaigns( flu clinics) through advice or direct care

    support delivery of QOF, incentiveschemes, QIPP and other quality or cost effectiveness initiatives

    perform other general tasks asassigned.

    Person Specification

    Qualifications

    Essential

  • Masters degree in pharmacy (MPharm) (or equivalent)
  • Mandatory registration with General Pharmaceutical Council (GPhC)
  • Desirable

  • Specialist knowledge acquired through post- graduate diploma level or equivalent training/experience
  • Independent prescriber or working towards/intent of gaining independent prescribing qualification
  • Membership of the Royal Pharmaceutical Society
  • A member of or working towards Faculty membership of the Royal Pharmaceutical Society
