We have two posts available in our commissioning team, which is structured based on the life course model of Start, Live andAgeing Well:
Start Well –Services and early intervention offer for Children and Young People (Residential, Supported Accommodation and Fostering, Special Educational Needs, and Transition)
Live Well – Services and early intervention offer for Adults with Disabilities aged 18 to 64 (Physical Disabilities, Mental Health, Learning Disabilities, Autism), and Public Health
Age Well – Services and early intervention offer for Older People aged 65+ (Dementia, Physical disabilities)
This is an exciting opportunity for an enthusiastic Project Officer, who has experience of collaboratively working with a range of key partners, to use their skills to set the direction of projects which will improve our offer to, and care of, some of the most vulnerable people within our Borough.
1 x 37 hours per week, permanent post in the Live Well Team~1 x 37 hours per week, 12 months fixed term contract in the Age Well Team
