    Description

    Position Overview: The company is offering an exciting opportunity for a role in the Haringey Memory Service (HMS) as a Service Provider. The main responsibilities include assisting in the assessment and treatment services for service users and carers referred to the HMS.

    • Participate and assist in the delivery of assessment and treatment services.
    • Administer components of the assessment and monitoring process.
    • Assist with post diagnostic interventions.
    • Undertake various duties involving direct service user assessment and monitoring.
    • Coordinate and run therapeutic groups.
    • Be knowledgeable about mental health issues in later life.
    • Provide culturally sensitive services to residents of Haringey.

    About the Company: The HMS located at St Ann's Hospital site is dedicated to serving the residents of Haringey by providing assessment, monitoring, and treatment interventions. The successful candidate will contribute to the ongoing quality of service and care identity by identifying any risk issues related to patients' health and social care.

    Job Responsibilities:

    • Provide culturally sensitive services to service users and carers.
    • Administer medication under supervision when required.
    • Offer information on health promotion and healthy living.
    • Demonstrate understanding of relevant legislations and acts.
    • Assist in the smooth running of the HMS.
    • Contribute to the quality of service and care by identifying risk issues.

    Person Specification - Education/Qualifications:

    Essential

    • Diploma in Mental Health Practice or equivalent experience.
    • Educated to GCSE level or equivalent.

    Desirable

    • Level 1 Safeguarding Children and Vulnerable Adults certificate.
    • Level 2 Safeguarding Children certificate.

    Experience:

    Essential

    • Experience in caring for older people with Dementia.
    • Previous experience in a Health or Social Care setting.
    • Knowledgeable about dementia and related services.
    • Understanding of the Mental Capacity legislation.
    • Knowledge of Advance Care Planning.

    Skills / Abilities:

    Essential

    • Excellent communication skills.
    • Ability to work independently and as part of a team.
    • Well-developed interpersonal skills.
    • Ability to form positive relationships with service users.
    • Proficient in using computers and keyboards.
    • Motivated and enthusiastic.

    Desirable

    • Awareness of patient care within the organization.
    • Knowledge of multi-professional team roles.
    • Experience in delivering recovery-focused interventions.
