    Main Job Duties:

    Join a team dedicated to helping individuals maximize independence and return or stay at home with the support of rehabilitation services. Patients have complex needs, and your role will involve a full-time commitment covering 36 hours per week, including day and night shifts.

    Job Responsibilities:

    Key Responsibilities:

    - Hold a degree or diploma and be a NMC registered adult nurse

    - Demonstrate expertise in Discharge to Assess, Community Urgent Care, Accident & Emergency, and rehabilitation

    - Collaborate with the multi-disciplinary team to support patients' home care needs

    - Conduct comprehensive assessments and interventions for clients

    - Lead in patient assessment, treatment, discharge facilitation, and home care support

    - Possess strong communication, organizational skills, and the ability to work independently under pressure

    - Benefit from a supportive induction program and secondment opportunities to broaden your experience

    - Embrace diversity and inclusivity in our recruitment process

    • Registered Nurse with current NMC registration
    • Experience in community nursing and IV therapy
    • Effective communication and team collaboration skills
    • Demonstrated ability to manage time efficiently and lead a nursing team

    Desired Qualifications:

    • Mentorship qualification or equivalent
    • Teaching experience in healthcare
    • IT Skills
    • Clinical supervision facilitator
