    Job summary

    An excitingopportunity has arisen for an enthusiastic individual to join our Housing,Adaptations and Specialist equipment (HASE) occupational therapy team. We providecomprehensive OT assessments relating to housing, major adaptations (DFGs),minor adaptations, and specialist equipment for residents of Worcestershire.

    This is a permanentpost, for hours a week, over three plus days with flexibility.

    We offer acomprehensive induction programme relating to major adaptations and DFGs. Weoffer and support continuing professional development to equip you with theskills and knowledge required for this post. We provide regular peer and 1:1supervision.

    Housing,equipment, and adaptations for all age groups is our specialism and the corearea of our work. This is an excellent post for someone wanting to join asupportive team withthe resourcesto develop and progress a long-term and rewarding career.

    Main duties of the job

    You willcarry out functional assessments within the home and, working with the residentand their carers, identify the challenges faced in remaining at home safelywhilst managing everyday tasks.

    Havingidentified the challenges, you will use robust clinical reasoning to makerecommendations in relation to the provision of major adaptations, housing andrehousing, specialist and none specialist equipment, and minor adaptations.

    You willwork in partnership with other health professionals, Local Councils, and theHome Improvement Agency to help deliver these recommendations with the aim ofpromoting independence, safety, quality of life and good health outcomes forresidents in Worcestershire.

    You will beresponsible for the recording, planning, and managing your own caseload.

    Whilstbased within a supportive team, you will have autonomy to balance and achievethe demands of this busy, varied but rewarding role.

    You will bean excellent communicator, working with clients, carers and other professionalsto achieve excellent outcomes for your clients. You will need to beself-motivated, have an excellent work ethic, be able to work autonomously,whilst being a team player.

    You willparticipate in the process of triaging referrals to the HASE OT Service toensure clients needs are being met by the most appropriate service and person.

    You will contribute to the smooth running of anoffice, team and to wider service development.

    Job description

    Job responsibilities

    Thepost holder will undertake occupational therapy assessments for a designatedcaseload, addressing occupational performance and skill deficits, enabling theclients to achieve their maximum independence.

    KeyResponsibilities Include:

  • Workwith clients to assess for major adaptations, specialist equipment and housingneeds
  • Liaisewith other health and social care professionals, families/carers, statutory andvoluntary organisations to enable people to continue to live in their own homes.
  • Contributeto the supervision of staff and students.
  • Workwithin the services quality standards and observe the policies and proceduresof the Trust.
  • Complywith the College of Occupational Therapy Code of Ethics and ProfessionalConduct and national and local policies and procedures.
  • Respectthe individuality, values, cultural and religious diversity of clients andcontribute to the provision of a service sensitive to these needs.

    Qualifications

    Essential

  • Degree / Diploma in Occupational Therapy
  • Health Professionals Council Registration
  • Evidence of relevant post graduate courses / Continual Professional Development maintained portfolio
  • Reflective practitioner
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer
  • Experience

    Essential

  • A range of post registration experience including in a health and social care setting
  • Experience of delegation to qualified/unqualified staff
  • Skills & Abilities

    Essential

  • A positive attitude and able to act on feedback.
  • Ability to communicate effectively in English in written format.
  • Excellent written communication skills
  • Knowledge

    Essential

  • A good level of knowledge in a wide range of physical conditions
  • Knowledge of the adaptation and Disabled Facility Grant.
  • High level of knowledge relating to disability equipment.
