Health Visitor – Band 6 – HRCHHounslow & Richmond Community Healthcare Trust are currently searching for a dedicated Health Visitor to work as part of their incredible Nursing team Childrens services team.
HRCH provides community health services for around 523,000 people registered with GPs in the London boroughs of Hounslow and Richmond.
The Trust currently employ around 1,300 people, who work across a wide range of health centres, hospitals, GP surgeries, children's centres, local council facilities and in community settings – including in people's homes.
This helps people across the community to stay well, manage their own health with the right support and avoid stays in hospital.
To work in a public health role in the provision of a comprehensive service to facilitate the improvement and maintenance of the health and wellbeing of children, individuals and families.
This will be achieved by applying the key principles of health visiting:# To search for health needs
# To initiate and contribute to strategies designed to promote and improve health and to encourage individuals to take responsibility for their own health
# Work collaboratively with all other agencies to safeguard and promote the welfare of children.
The post holder will carry overall responsibility for individuals, families and communities in line with contractual agreements and within their sphere of responsibility.
Working in collaboration with members of the Primary health care team, allied health professionals and other key members of statutory and voluntary agencies are key components of this roleTo assess the health needs of individuals, families and communities.
To plan and implement programmes that promote and protect health such as screening programmes, health promotion campaigns, immunisations, planning and delivering integrated services across agencies.
To actively promote immunisations/vaccinations and to engage in opportunistic immunisation.To work as part of a corporate health visiting team.
To organise and conduct Well baby clinics in Children's Centres and other community settings.
To identify health inequalities and take appropriate action to address these, by working collaboratively with health, statutory and voluntary agencies to address the wider determinants of health (i.e.) housing, transport and social exclusion.
To work with communities and users of the service to develop and design services to meet identified needs and support family health assessments where there are mental health problems.
This should be achieved through collaborative and partnership working with key health professionals and statutory and voluntary agencies.To identify and assess situations where children are subjected to abuse and children who are looked after. Be able to monitor those families and be engaged in subsequent care and monitoring process. This will be done in full accordance with local and national child protection policies.
To identify, assess, plan a programme of care where there are children with disabilities and special needs. To advise and refer to the Specialist health Visitor for Children with special needs.
To advise and refer to the Specialist health Visitor for the homeless any homeless and vulnerable families (i.e.) refugees and asylum seekers of services available through the NHS and other statutory and voluntary agencies.
