    Description
    An exciting opportunity has arisen at Prescott House Community Mental Health Team in Salford.

    We are looking to recruit a Band 6 Community Psychiatric Nurse to work within the Teams to cover a secondment, approximately 12 months.


    We are looking for experience of working within mental health ina community setting, however consideration will be given to candidates who have experience and transferable skills from other areas.

    This post provides a challenging opportunity to work within an established fully integrated supportive Community Mental Health Team.

    We provide a supportive working environment to assist with the transition from inpatient nursing to community. An extended induction period also helps nursing staff to feel supported in the community role.

    You will hold and manage a caseload of service users and have Care Coordinating responsibilities under the Care Programme Approach Guidelines.

    By assessing your service users and carers needs you will be able to access and sign post to a wide range of community resources and provider services available in Salford.

    The post holder will be expected to participate in providing quality first line psychosocial assessments as part of GMMH's Functional CMHT's duty role and work proactively in forging professional working relationships with multi-agencies.

    There is an expectation to work weekends /BH's as part of GMMH's Acute Care Pathway provision of care. This is on a rota basis between the four community teams.

    Nursing Qualification
    Nursing experience
    Experience working as a qualified nurse in mental health
