    Practice Development Nurse (Childrens Community Nursing)

In order to help achieve this ambition we are looking for Practice Development Nurse to join the CLCH Academy to promote and facilitate practice development across our Children's Community Nursing and Special School Nursing teams, in our North West London and North Central London Division. As a PDN you will join the CLCH Academy Team where you will be supported with your development whether this is at undergraduate or post graduate level.

    • Promoting high standards of care through the demonstration of clinical expertise
    • Facilitating the creation of a positive learning environment in the workplace where staff development is encouraged and supported
    • Facilitating the induction, education and continuing professional development for all levels of registered and non-registered nursing staff within the Childrens Community and Special School Nursing Teams
    • Working alongside staff in individual teams undertaking a key role in practice development
    • Translating Academy priorities such as changes in nursing standards, new clinical roles such as Apprenticeships into their designated service

    • To establish and maintain productive working relationships and communications with key stakeholders
    • To contribute to relevant internal and external committees and forums, and represent the Academy team or designated service as required
    • To provide progress reports aligned to measurable project deliverables to the Academy team, Locality Manager, divisional staff, and other key stakeholders
    Planning
    • To work with the Academy team and Locality Manager to identify specific training needs and plan programmes to ensure staff are competent and have the right skills to undertake their roles
    • To work with the Academy team, nursing teams, CBU Manager, Divisional Director of Nursing and Therapies as an ambassador for practice development
    • To support community team leaders in planning, implementing and evaluating practice changes in line with evidence and best practic
    Implementation
    • To work with nominated team members, Clinical Leads and CBU manager to identify, plan and implement initiatives within teams
    • To link with internal leads including the transformation team, human resources and the Academy team in planning, implementing and evaluating actions and outcomes
    • To implement a process of effective communication within teams, ensuring that staff are able to inform developments and contribute to chang
    Monitoring
    • To develop metrics for monitoring the delivery of training and development objectives and ensure the information systems are accessible for robust and regular monitoring
    • To take responsibility for the development and maintenance of the information systems that inform performance monitoring against the key milestones
    • To monitor progress against key milestones and work with teams and individuals to resolve any challenges to achievement
    • To assist staff in the evaluation and publication of outcomes both internally and externally
    • Using designated metrics, determine areas where changes in practice require policy and protocol change or development and lead the changes with the relevant nursing teams
    Communication
    • To develop a robust communication plan for nursing staff for any training and development initiatives
    • To maintain liaison with other key external stakeholders including the Royal College of Nursing, the Queens Institute, Commissioners and primary care staff
    • To link to and contribute to the work of the Academy
    • To provide briefings on national and local workforce and development initiatives
    • To promote the work of the Academy with internal and external stakeholders
    • To participate in local and sector wide forums on behalf of the trust relating to key work streams so as to inform wider strategy and champion local implementation
    • To participate in research and audit activities that provide robust theoretical and conceptual frameworks in collaboration with academic institutions
    Clinical Practice and Clinical Leadership
    • To work in accordance with the NMC Code of Professional Conduct, and Trust Policies and Procedures
    • To ensure that own clinical practice demonstrates current clinical knowledge
    • To work clinically within nominated teams on a regular basis, supporting teams and individual nurses in developing their practice
    • To enable staff to identify opportunities to develop clinical practice, and support them to make changes in clinical practice
    • To enable access to resources and evidence to support the development of clinical practice within nursing
    • To embed a sustainable approach to the provision of clinical supervision within the designated service
    • To support wider clinical and Academy imperatives including the implementation of the care certificate
    • To support clinical supervision within and across service
    Education and Training
    • To participate in research and audit activities that provide robust theoretical and conceptual frameworks in collaboration with academic institutions
    • To create, sustain and support an environment where practice development and change can flourish
    • To identify practice supervisors and assessors and preceptors and support them in their role, ensuring that learning opportunities are effectively utilised
    • To embed approaches to competency based practice within teams
    • To link with wider stakeholders in relevant training and development of staff
    • To plan training and educational input for relevant groups of staff in collaboration with the Academy team, professional leads and heads of clinical services
    • To develop and co-ordinate planned educational programmes in line with the Academy objectives identifying opportunities for nursing and support staff
    • To enable action learning for staff within nursing
    • To participate in Clinical Fridays and other key quality activities
    Financial Management
    • To contribute to achieving the overall financial goals of the organisation and the monitoring and effective use of resources
    • To identify potential funding opportunities and lead the development of bids for funding
    • To work with the Academy team to manage and monitor contracts with educational institutions and training provider
    Leadership
    • To act as an effective professional role model to others
    • To support the leadership development of team and deputy team leaders, aligned to the trusts leadership strategy
    • To support the development of the CLCH Academy across the organisation and with external stakeholders
    • To work collaboratively with key stakeholders across the Trust (Directors, Service Managers, Community and Modern Matrons, Nurse Consultants, Clinical and Non-Clinical frontline staff) to enable the implementation of clinical and service development
    Strategy
    • To contribute to the corporate strategic and business agenda by promoting a culture of quality, learning, performance and development
    • To work closely with the Academy team to ensure that the Academy priorities are aligned to the local service need

    • To act as a resource to staff, providing advice, guidance and resources relating to relevant projects and initiatives
    • To work with Human Resources leads and other key stakeholders to develop strategies which enhance staff recruitment, career development, workforce objectives and staff satisfaction
    Physical Mental and Emotional Effort
    • The post holder is required to frequently use Visual Display Equipment, computers and laptops Frequently travel to strategic meetings with partner organisations
    • Exposed to frequent long periods of intense concentration analysing data and reports and prioritising with tight deadlines both locally and nationally
    • Deal with conscientious issues including working with patients, families and service users, involving staff, objective setting, monitoring and development

    Person Specification Education/Qualification Essential
    • Registered Nurse
    • Educated to Masters degree level or equivalent
    • Clinical Leadership training

    • Significant senior experience within Children's Community or Special School Nursing Services
