In order to help achieve this ambition we are looking for Practice Development Nurse to join the CLCH Academy to promote and facilitate practice development within our community nursing teams. As a PDN you will join the CLCH Academy Team where you will be supported with your development whether this is at undergraduate or post graduate level.

    • To establish and maintain productive working relationships and communications with key stakeholders
    • To contribute to relevant internal and external committees and forums, and represent the Academy team or designated service as required
    • To provide progress reports aligned to measurable project deliverables to the Academy team, Locality Manager, divisional staff, and other key stakeholders
    • To work with the Academy team and Locality Manager to identify specific training needs and plan programmes to ensure staff are competent and have the right skills to undertake their roles

    • oRegistered Nurse
    • oEducated to Masters degree level or equivalent
    • oClinical Leadership training
    • oEvidence of continuing personal and professional development

    • oLearning and Development Qualification
    Experience Essential
    • Significant senior experience within district nursing services
    • Previous experience in practice development
    • Experience in taking forward clinical and service change
    • Experience of working in a changing and uncertain environment.
    • Experience of delivering on development initiatives and leading change with individuals and teams.
    • Experience of working with staff and stakeholders at all levels.
    • Experience of using evidence to inform best practice
    • Experience of engaging with patients, the public and stakeholders
    • Experience of report writing and presentation
    Skills & Knowledge Essential
    • Knowledge of the national priorities relating to provision of district nursing services
    • Skills in articulating a vision to staff working at different levels
    • Knowledge of practice development and change management.
    • Knowledge of the NHS agenda of how it impacts on service delivery
    • Ability to think strategically. Develop and deliver strategies and plans.
    • Ability to implement and evaluate successful change programmes.
    • Leadership skills, relationship building skills and influencing skills.
