    Sister/Charge Nurse – Transfer of Care Team Oldham Community Services - Northern Care Alliance NHS Foundation Trust

    Job summary

    An opportunity has arisen for an experienced Band 6 Sister / Charge Nurse to join an established Integrated Discharge Team / Transfer of Care.

    You should be able to demonstrate the skills and knowledge of working within a multidisciplinary team setting, but equally have ability to work to your own initiative.

    Working flexibly between Health and Social Care you will provide specialist assessments to facilitate the discharge of Oldham residents into Intermediate Care, Palliative Care or Discharge to Assess beds in Residential Care and Nursing Homes.

    Main duties of the job

    The post holder will work in partnership with colleagues across health and social care, in and out of hospital, to facilitate the safe discharge of patients with complex care and palliative needs from Acute Hospitals, in order to reduce the length of stay and improve outcomes for patients and their carers. To co-ordinate and manage the discharge process for patients with complex health care needs includingcomplex continuing care assessment, ensuring a consistent approach to the application of criteria.

    To provide specialist advice on community/primary care services to enable the multidisciplinary team to make an informed risk assessment, with regards to the patient returning to a community setting wherever possible and/or to expedite a patient's transfer/discharge.

    Co-ordinate the holistic assessment of patients with complex health care needs. Liaising with members of the multidisciplinary ward team, in order to affect a safe and timely discharge. To maintain strong collaborative working between health/social care and other key stakeholders. To work closely with the palliative teams across the sites to facilitate fast track discharges for patients in the last days or weeks of their life. To work in collaboration with the continuing health care team to apply for funding for fast tracks and support the screening process for continuing nursing care.

    This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months. Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.

    Qualifications / Experience

    Essential

  • NMC registered
  • Substantial post registration
  • RGN - Pin Number to date
  • Evidence of having undertaken relevant further study or training as required for the post
  • Commitment to integrated working across health and social care.
  • Ability to plan and prioritise workload.
  • Experience of complex discharge co-ordination.
  • Knowledge of DOH guidance and legislation in relation to discharge and transfer of care and DToCs.
  • Knowledge and experience of the co-ordination of continuing health care assessments and application of criteria.
  • Desirable

  • Evidence of Continuing Professional Development
  • Recognised management or leadership qualification
  • Ability to provide leadership to junior members of the team.
  • Good interpersonal skills with the ability to communicate with a range of people.
  • Ability to motivate and influence colleagues within and outside of the Team.
