    Main duties of the job

    What youll be responsible for

    The post holder is responsible for the pre-operative anaesthetic and medical assessment of patients being prepared for a variety of surgical procedures.

    Provide a comprehensive anaesthetic and medical assessment, including baseline outcome measurement, ensuring that all necessary investigations ( follow NG45 with local guidance) and results are collated prior to the patients admission.

    Liaise with Anaesthetists/Surgeons regarding potential medical problems and issues regarding surgery. This role will also include appropriate onward referral to primary and secondary care providers to ensure pre-operative health is optimised.

    To liaise closely with clinic administrators regarding booked admissions, cancellations and DNAs.

    To liaise closely with ward managers, consultant surgeons and anaesthetic staff, nurse specialists, dieticians, therapists, discharge coordinator and MDT regarding complex admissions and patients with potential discharge problems.

  • Main Responsibilities:

  • To provide specialist nursing advice and support to patients, their families and other healthcare professionals from point of decision to carry out surgical intervention through to admission.
  • To actively contribute to the reduction of length of stay.
  • To carry out detailed preoperative assessment of elective patients, including initiation of discharge planning procedures.
  • Perform and document medical and nursing pre-operative assessment. This includes patient history, health screening and carrying out a medical examination. Give pre-operative advice and support and initiate appropriate referral to other members of the multi-disciplinary team, as part of the patients surgical pathway.
  • To carry out/ initiate preoperative investigations/diagnostic tests in line with existing pre-assessment protocols & NICE guidance.
  • Exercise clinical judgement and decision making about patients needs in terms of suitability for surgery, consulting Clinicians, as necessary.
  • To be responsible for following up and reviewing investigation results and alerting appropriate clinicians, as necessary.
  • Liaise and/or refer to primary or secondary care providers to ensure pre-operative health is optimised.
  • Promote lifestyle changes to patients, as indicated, weight loss, smoking cessation.
  • To work autonomously by accepting responsibility for own patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations made ensuring clear documentation in the patients records (hand held and electronic records)
  • Liaise with multi-disciplinary teams on the provision of the service to ensure the highest quality of patient care. Provide clinical advice to other multi-professional team members in pre-operative assessment.
  • Ensure that evidence based written patient/carer information is compiled, as necessary, and available for patients to ensure they fully understand the proposed surgery and anticipated post-operative recovery.
  • Continue the informed consent process by ensuring the patient is aware of the risks and benefits of the procedure.
  • Initiate discharge planning and identify any potential problems that may delay discharge prior to admission. Refer to appropriate primary and secondary services, as necessary. Provide patient and carer with realistic estimate length of hospital stay and advice on arranging home circumstances following discharge.
