    Job summary

This exciting opportunity to work with the complex needs team as an Improvement Facilitator will mean you will be at the forefront of trustwide improvement projects to support us to ensure patients with a complex need receive excellent patient experience each and every time.     Main duties of the job

    You will support the Complex Needs Team by working alongside clinical and non-clinical colleaguesto make improvements in relation to quality and patient experiencefor those with complex or enhanced care needs.

    You will support the development and roll out ofquality improvement projects as well as assisting in preparation, assurance,and response to the Complex Needs forward plan and the Complex Needs governanceaction plan.

    You will develop andprovide overall project management support on a number of quality and safetypriorities in relation to Complex Needs and Enhanced Care.

    You will also be supporting effectivecommunication and stakeholder management, both internally and externally, working closely with those with lived experience.

    Job description

    Job responsibilities

    The post holder provides management support to the Complex Needs Team. This will involve working alongside clinical and non-clinical colleagues to ensure they have timely information in relation to quality and patient experience for those with complex or enhanced care needs.

    Complex Needs is a term the Trust use to describe learning disability, autism, dementia, delirium, mental health.

    The post holder will also work with various members of staff Trust wide in the development and roll out of quality improvement projects as well as assisting in preparation, assurance, and response to the Complex Needs forward plan and the Complex Needs governance action plan. Support teams to ensure that the portfolio of tasks/projects is planned, managed, and delivered effectively.

    The post holder will develop and provide overall project management support on a number of quality and safety priorities in relation to Complex Needs and Enhanced Care including the complex needs forward plan, Complex Needs improvement projects, and complex needs governance framework.

    Be a key member of the team as well as supporting effective communication and stakeholder management, both internally and externally.

    Assist in the development and roll out of quality improvement projects for the Governance Framework, Patient Experience and Patient Safety, related to complex needs.

    Assist the Complex Needs team with gathering evidence, preparation of reports and analysis of data. This will involve office-based work as well as ward-based audit and education.

    Support clinical and non-clinical teams to make improvements in Quality of Care through the use of available data from key data sources such as Complex Needs governance Framework, local patient survey results, working with experts by experience, and Trust complaints.

    Support a variety of campaigns across the Trust , this will involve the use of social media , preparation of campaign materials and visiting clinical areas to support staff in the cascade and implementation of the improvement projects.

    Support the organisation and administration of the complex needs teams meetings with wards.

    Escalate any areas of concern or risk to the Complex Needs Lead Professional or Head of Complex Needs.

    Analyse and interpret complex data and produce reports for a various meetings.

    Use a variety of methods to interrogate both quantitative and qualitative complex information and present this information in a meaningful way.

    Support the Complex Needs Clinical Educator and Lead Professional to share results from audit and deliver regular reports to relevant stakeholders.

    Work alongside the Complex Needs team to triangulate quality and patient experience data with other sources of information.

    Produce and present reports detailing trends and themes, lessons learnt and resulting changes in practice for directorate or divisional meetings and for quarterly and annual reports.

    Support the Complex Needs team to write professional reports displaying relevant information to a range of committees and stakeholders.

    Sit on / attend relevant groups undertaking administrative tasks and contributing papers, reports, and results.

    Support the Complex Needs Team to promote quality improvement activity within Divisions.

    Support the Complex Needs team in the development of creative approaches to sharing learning on service improvements.

    Demonstrate effective communication with colleagues to ensure team relationships are developed.

    Ensure timely exchanges of information to other departments and external bodies.

    Contact patients with complex needs to identify information that can support an improved journey through the Trust. contacting patients to find out what reasonable adjustments may be needed to support their attendance at hospital.

  • Significant experience in undertaking systematic approaches to quality and patient experience improvements.
  • Experience of working with groups to improve patient experience and quality.
  • Professional report writing.
  • Experience of at least one of the Complex Needs or Enhanced Care.
