    Calling band 7 mental health nurses

    Would you like to join us in an exciting role as the Clinical Lead for the Care Home Pathway within a well-established CMHT for older people based in NW Surrey (Unither House, Chertsey)?

    We are keen to recruit an experienced Band 7 Community Psychiatric Nurse to provide specialist input to the Care Home Pathway within the team. You will be working alongside colleagues from a range of professional backgrounds (Psychology, Occupational Therapy, Nursing, Assistant Psychologist and Associate Practitioners) to provide specialist advice and consultation to people living in care homes within the local area. This will predominantly focus on people with dementia who are showing behaviour that challenges themselves or others, and may also include people with moderate-severe mental health difficulties.

    We are keen to meet individuals with a passion for learning new skills, particularly in the field of behaviour that challenges in dementia care.

    You will be encouraged to attend our excellent in-house academic programmed, complex case discussions and staff support forum, both led by the psychologists. Surrey and Borders NHS Foundation Trust also has a supportive Nursing network that you can benefit from, which includes access to a Nurses Forum, rolling internal CPD programmes, Non-Medical Prescribing training.

    It is essential that you have a full, valid UK driving licence and personal access to a vehicle for work-related purposes.

    Main duties of the job

    You will provide holistic, autonomous care and maintain communication with primary care professionals and other professionals and agencies through regular informal and formal meetings. These meetings will include a review of patients within the practice/service with mental health problems.

    You will facilitate clinical sessions and assess/triage clients/patients according to agreed assessment tools, offering short-term interventions based on psychological therapies and offer professional advice to client/patients and their carers, other professional, statutory and voluntary agencies.

    You will facilitate the transfer of patients between primary and secondary care and between the different specialist teams within the Trust and to care coordinate clients on both standard and enhanced CPA.

    You will lead on the setting, monitoring and maintaining standards of care and will advise the sector manager on issues which endanger standards of practice within your specific area of practice.

    Job responsibilities

  • Responsible for a defined complex caseload within the team. Using specialist Mental Health knowledge and skills, carry out individual clinical assessment, plan and formulate a range of interventions and treatment options within care plans, implement and evaluate packages of care.
  • Undertake the task of care coordinator, participate in CPA and Caseload review meetings, discharge planning meetings involving service users, their carers, other professionals and agencies, attend inpatient reviews and act as client advocate if necessary.
  • Facilitate the development and use of care pathways by the multi-disciplinary team.
  • Treat clients with short term interventions where assessment has indicated this. Assess and treat clients with complex, severe and enduring mental health problems who may be on a community section of the mental health act.
  • Facilitate the transfer of client from Primary Care to Mental Health Services as appropriate and discharge clients from case load to appropriate agencies when clinical interventions are completed.
  • Act as a support and specialist advisor to GPs and primary care colleagues as appropriate.
  • To facilitate and develop with Primary Care Staff, joint education and training programmes of mental health issues in primary care.
  • To be part of the Senior Staff Group and attend relevant regular meetings as scheduled.
  • Ensure that effective communication systems exist between the team, Primary Care and wider Integrated Care service.
  • Ensure information is collected in such a way that it conforms to the Trusts and Social Care quality data required.
  • Work with team colleagues to achieve and maintain Quality Standards
  • Participate in foundation standards and service accreditation programmes
  • Carry out agreed clinical procedures and treatments in accordance with agreed policies.
  • To attend Tribunals and other Multi-Disciplinary Meetings and prepare reports.
  • To supervise and participate in the safe carriage, storage and delivery of medication to clients both in the clinical area and domestic environment, according to Trust and National Guidelines and Policies, ensuring that ordering and delivery of clinical equipment meets policy requirements.
  • To act as practice assessor and practice supervisor or delegate appropriate mentor for clinicians from other clinical areas wishing to gain experience in Team.
  • To ensure that carers are in receipt of Carers Assessments in accordance with the Carers Act.

    Qualifications

    Essential

  • A clinical qualification to degree level or equivalent.
  • Membership of a professional body (NMC)
  • Must own a vehicle for work purposes
  • Desirable

  • Passionate and committed and who enjoys working with people living in Care Homes, managing staff well-being and supporting people to achieve their potential.
  • Experience/ Interest/ skill in specialist work CBT, Psychosocial Interventions
  • Non-Medical prescriber
  • Experience

    Essential

  • Have a minimum of 5 years' experience working with people with MH needs, 2 of which must be at a Band 6 Community Mental Health experience
  • ENB 811, 812 or equivalent Community Nursing course
  • Desirable

  • Older Adult mental health experience
