    Job summary

    An exciting opportunity has arisen for a Trainee qualified ACP tojoin the Hereford City Community Mentalhealth Neighbourhood Team based at Widemarsh Street. We are a forward thinkingservice delivery model that welcomes innovative thinkers and values itsemployees and people with lived experiences who are accessing our service.

    This is an exciting opportunity.The ACP trainee will contribute to the support and development of theteam, and through development use the four pillars to influence and underpinservice delivery and design. You willbe supported within the team by both the clinical and leadership structure includingthe ACPs during the training. You will be shaping our services and ensuringthat ultimately our patients and service users get expert care focused onrecovery and independence from a non-medical prescribing provision, whilstworking as an autonomous advanced practitioner who will complement the serviceprovision.

    Full time post hours per week, over 5 days.

    Main duties of the job

    The post holder will work with a degree of professionalautonomy appropriate to level of training and begin to make critical/complexdecisions with the support of qualified advanced clinical practitioners and themedical team.

    Undertake advanced clinical assessment appropriateto level of training of patients who have complex and or chronic diseasepresentations, including screening patients for disease risk factors and earlysigns of illness, making a differential diagnosis, prescribing treatments as anindependent non-medical prescriber, initiating and reviewing treatment plansand discharging patients.

    Working with a range of agencies internally and externally,signposting to appropriate sources of support providing short to long-terminterventions to support recovery. Provide guidance and support to the carersof patients accessing the services and our partners.

    To be able to travel throughout the county asrequired. To supervise junior staff and students. Theability to be flexible and organised is essential.

    Job responsibilities

  • Work with a degree of autonomy and complex decisionmaking
  • Undertake advanced clinical assessment appropriateto level of training.
  • Work collaboratively and cooperatively withclinical colleagues to develop services and quality of care delivered byproviding clinical knowledge and skills and undertaking advanced clinicalpractice with an appropriate level of supervision
  • Develop and maintain effective communicationsystems to ensure staff receive and contribute information regarding Trust andprofessional issues and developments.

  • To attend and chair meetings required by theservice

    Essential

  • Demonstrable post graduate experience in more than one clinical area including that which applying to
  • Be able to demonstrate some experience in each of the four pillars of the Advanced Clinical Practitioner role Clinical Practice, Leadership, and Management, Education and Research
  • Experience of working with people with long term illness and/or frailty/ older people
  • Clinical experience of caring for people with cognitive impairment
  • Experience of working across professional boundaries
  • Experience of participating in change/service development
  • Experience of clinical leadership
  • Knowledge

  • Extended and developing advanced clinical skills and knowledge base
  • Understanding of professional and current issues in relevant clinical area
  • Demonstrate an understanding of clinical governance
  • Understanding of relevant legislation and guidance relating to clinical area
  • Understand the framework for working at an advanced level
  • Knowledge of theory behind change management
  • Qualifications

    Essential

  • Registered Nurse Level 1/Allied Health Professional/Paramedic/ Pharmacist
  • Degree level qualification
  • Undertaking MSc in Advanced Clinical Practice or have the experience and qualifications to undertake MSc in Advanced Clinical Practice
  • Evidence of a portfolio of knowledge, skills and professional development
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer
  • Desirable

  • Mentorship or teaching qualification / work place assessor
  • Qualification(s) in long term conditions
