Job Summary
To provide an effective clerical and admin service for the In-Patient Unit. The post holder is the first point of telephone contact and responsible for providing an efficient and helpful service. The post holder will work in the Nurses' Station and Day Hospice.


Main Duties & Responsibilities

  • Monitoring and maintaining supplies of patient notes sets and other patient documentation (charts, record sheets) kept in the Nurses' Station and In-Patient Unit office. Photocopying and compiling additional supplies of patient notes sets, as required.
  • Sort, collate, file, scan and store all patient information, when signed, in the correct order.
  • Arrange patient investigations, treatments, transfers, and discharges. Occasionally arrange transport through other means as necessary.
  • Request and return hospital notes, as required by Medical and Nursing Staff, liaising with medical records departments, following up when the location of records is not straightforward. Medical examiner notes to be collected and returned.
  • Maintain comprehensive records of all action taken with regards to patient records.
  • Answer the telephone in the Nurses' Station and In-Patient Unit office to ensure effective communication of patient information. Calls will be from a wide range of people including relatives, other medical professionals, and Funeral Directors. Confidentiality must always be maintained, and timely, accurate communication of information is crucial.
  • Administration support for the In-Patient Clinical Staff in the In-Patient Unit and Day Hospice, e.g. patient letters, appointments, statistical requirements, patient information.
  • Compile and maintain uptodate information on medical and Allied Health Professionals' contact telephone numbers for use in the Nurses' Station.
  • Monitor and maintain supplies of stationery and patient information leaflets, compiling additional packs, as necessary.
  • Inform all relevant Health Care Professional and Medical Staff following certification of death and ensure completion of Notification of Death form for the CQC.
  • Complete routine filing of all patient records.
  • Responsibility for maintaining the In-Patient bed management patient database to ensure the correct recording of bed occupancy for the In-Patient Unit.
  • Responsibility for ensuring all the demographic details are entered on the patient database for all patients in the In-Patient Unit.
  • Production of discharge and death proformas for completion by Doctors and ensuring they are sent as soon as possible and with required timescales.
  • Undertake other admin and support tasks as required by the In-Patient Unit Team Leader or Nurseincharge and Day Hospice.
  • Assist with patients' nonmedical needs as directed by appropriate clinical/medical staff
  • Ensure Clinical Audit Tracker data maintained.
  • The post holder will be involved in the nonclinical care of the dying/deceased patient and their families and carers. This will encompass the physical, psychological, and spiritual aspects of the dying patients' care and when the patient has died.
  • Liaise with community healthcare teams e.g. G. P's, MacMillan Nurse Team, Advance Care Planning Team, Family Support, Quality Assurance Team as required.
  • Maintain annual servicing of service records for Medical equipment of syringe drivers, suction pumps and any other equipment as directed as necessary.
  • Ensure monthly statistics are provided for CCG, CEO and any other as required
  • Collecting, compiling, and submitting CCG Halton figures to QA (monthly, quarterly, and annually)
  • Collecting, compiling, and submitting CCG Cheshire West figures to QA (monthly, quarterly, and annually)
  • Providing data for audits and friends and family test (monthly)
  • Providing data for investigations up to and including Serious Incidents and Root Cause Analysis
  • Responsible for the filing, storage, and disposal of confidential records in accordance with Hospice policy and legal requirements
  • All staff are required to attend inservice training.
NB. QA works very closely with Ward Clerk position to collect information quickly and efficiently on a regular basis to complete CCG reports

  • Use all work equipment (medical devices, dangerous substances, machinery, transport, means of production, and safety equipment) in accordance with training and instructions provided.
  • Not use such items or equipment unless they have received appropriate information and training.
