    We have the opportunity to recruit to a band 7 post for a 24 month contract in the Community Sickle Cell and Thalassaemia team:

    We are seeking to employ RN registered nurses with strong clinical experience.

    This post will be based within a well-established nursing team working across Southwark, Lambeth, Lewisham, Bexley and Greenwich.

    We are looking for applicants who are keen to learn, can work independently and have a passion for improving the lives of patients living with Sickle Cell.

    Our CNS team work with a diverse patient demographic providing a range of support including antenatal support, new-born visits, creation and reviews of care plans, support for patients discharged from hospital after admission.

    Main duties of the job

    The post holder will lead and manage nurses to develop and deliver personalised care packages for individuals with sickle cell and thalassemic conditions.

    You will manage a caseload of patients, ensuring each patient has an up to date care plan, conduct home visits, and link in with other care providers to provide wrap around support for patients and ensuring their safety and well being.

    Essential criteria

  • Advanced clinical and leadership skills in specialty.
  • Proven listening / counselling skills with the ability to manage complex situation and appropriately deliver sensitive and difficult outcomes.
  • Evidence of teaching skills in particular teaching patients in how to manage their conditions and raise their awareness of their condition
  • Ability to clinically influence staff
  • Specialist knowledge of the medical, nursing, social and psychological implications of sickle cell and thalassaemia
  • Understanding of basic assessment of mental health
  • Excellent verbal, written, interpersonal & communication skills
  • Able to use skills of history taking and physical assessment to direct patient care
  • Time management and organisation skills
  • Ability to conduct & evaluate own projects successfully
  • Demonstrate an understanding of: the National agenda for speciality. Knowledge of quality standards & NICE recommendations/NSF

  • Extensive experience of working autonomously at an advanced level within the specialist area of haemoglobinopathies, delivering effective patient focused care.
  • Significant post qualification experience of in the community and demonstrable experience of interagency and collaborative working
  • Previous experience of dealing with safeguarding issues
  • Proven evidence of autonomous practice
  • Experience of delivering patient advocacy
  • Experience of outreach and health promotion
  • Knowledge and experience of delivering culturally sensitive services
  • Qualifications/ Education

    Essential criteria

  • BSC/1st Degree (helath related) or equivalent qualification/training and experience
  • Post registration qualification in appropriate area of speciality or equivalent post qualification experience.
  • ENB No8 Care and Management of Individuals with Sickle Cell and Thalassaemia (or willingness to undergo relevant training)
  • Certificate in Counselling
  • Evidence of professional development
  • Training in safeguarding
  • Registered General Nurse (RGN)
  • NMC Registration
  • Desirable criteria

  • Certificate in Mentoring
  • Course in High Dependency Care
  • Course in Wound Management
  • Registered Nurse Prescriber (or willingness to undergo relevant training)
  • Professional/ Statutory Registration

  • RN1 or RN on the NMC register. To hold the appropriate level for the area of speciality
