    An exciting opportunity has arisen to join a new, innovative and dynamic team within our expanding virtual wards. Working into the frailty virtual ward in the community, we are looking for experienced and motivated Band 8A Lead Nurse/AHPwho are passionate about service development and providing high standard of patient centred care. You will be working closely within a Multi Disciplinary Team of staff including Doctors, Nurse Practitioners, Occupational Therapists, Physiotherapists and HCA's, all of whom are committed to maintaining independence, promoting self-care and assisting patients in their usual place of residence. At times this post may require you to work autonomously, when working across the city into patients own homes.

    As a Lead Nurse/AHP, you will use your clinical decision making through assessment and interpretation of clinical information to support frail and elderly patients having an acute exacerbation of a frailty related condition, and proactively case manage patients requiring frailty advice, addressing their unmet needs and improving their care coordination and wellbeing. Leadership qualities are necessary to support both the service delivery, and staff support and work allocation.

    Main duties of the job

    Newham University Hospital Frailty Virtual Ward plays a pivotal role in supporting patients with acute frailty syndromes at their usual place of residence, both supporting early discharge and preventing hospital admission. The geriatrician-led team enables the medical management of acute illness and frailty syndromes with the patient supported by their familiar environment by familiar carers, supplemented by additional members of the frailty multidisciplinary team as needed.

    Patients will be referred to the Frailty Virtual Ward from 'Front of House' (Emergency Department (ED), ward step downs (usually frail wards) and Community locations. Following initial assessment to confirm diagnoses, eligibility and both acute and long -term care needs, patients will be admitted to the Frailty Virtual Ward and a multidisciplinary management plan devised and communicated to patient and carers. This will involve interventions from across the team, including yourself as a specialist community nurse, and geriatricians, physiotherapists, occupational therapists, dementia and delirium specialist nurses, pharmacists and health care assistants (HCA)

    Essential criteria

  • Registered nurse/AHP
  • Degree in relevant health area
  • Relevant training/qualification in a specific area of neurosciences/ older peoples services.
  • Evidence of continuing professional development activity to maintain up to date knowledge and competence in all aspects of the role
  • Foundation knowledge of service improvement methodologies
  • Desirable criteria

  • Full driving licence
  • Experience/ Knowledge

  • Minimum of 5 years post registration experience working in a relevant area of clinical practice
  • Experience of managing a clinical team including performance management and appraisals
  • Experience of service development and improvement

  • Experience of working in culturally diverse environment

  • Minimum of 2 years of Clinical Research experience in NHS, University and/or commercial clinical trials environment
