    Job Description - Band 6 Nurse at Camden Virtual Ward

    An amazing opportunity is now available for a Band 6 Nurse to join the Camden Virtual Ward team. Your main mission will be to provide in-depth holistic assessments for patients dealing with acute frailty syndromes.

    Camden Rapid Access Service (Camden RAS) is a service that focuses on admission prevention and early discharge. It encompasses three pathways: Urgent Community Response, Discharge to Assess, and Virtual Ward.

    In this role, you'll be delivering nursing care to patients facing complex conditions like Frailty, delirium, and dementia. This opportunity will significantly contribute to your professional growth due to the diverse cases you'll handle.

    Reporting to the Virtual Ward manager, you'll be in charge of evaluating and managing the health needs of the target population. Your responsibilities will involve developing and implementing personalized care plans within a multidisciplinary team.

    • To operate within the established framework for Camden Virtual Ward, ensuring the provision of comprehensive acute care, guidance, and information to patients and their caregivers
    • Conduct thorough holistic assessments, formulate plans, implement interventions, and evaluate care plans to address patient's needs
    • Deliver proficient nursing care, perform clinical procedures, and maintain accurate records
    • Promote respect for privacy, dignity, and confidentiality in all interactions

    Lone Working

    This role may involve working alone in patients' homes or the community, including evening hours. Safety measures are in place to ensure a secure work environment.

    Job Responsibilities

    The ideal candidate will have advanced physical assessment skills and be able to independently perform top-to-toe assessments, escalate using NEWS 2 score, and take comprehensive patient histories.

    Being forward-thinking and able to prioritize based on clinical need is crucial. Acute care skills are essential, and a background in acute frailty, emergency, or intensive care settings is preferred.

    You will be responsible for managing patients needing Virtual Ward assessments and delivering clinical leadership within the team.

    Education and Qualifications

    Essential

    • Registered Nurse with current NMC registration
    • Minimum of BSc (University-based education)
    • Evidence of continued professional development

    Desirable

    • Mentorship qualification or relevant courses
    • Physical Assessment Course or related courses

    Experience

    Essential

    • Substantial experience in acute care
    • Understanding of Virtual Ward operations

    Desirable

    • Community working experience
    • Specialized training in frailty

    Other

    • Awareness of NHS primary care measures and initiatives
