Therapist/Allied Health Professional
An exciting opportunity has arisen within the Admission Prevention Team based at the Royal Sussex County Hospital for an experienced therapist/Allied Health professional.
The post holder will work within the A& E department based at the Royal Sussex County Hospital.
The post holder will act as an ambassador for Sussex Community NHS Foundation Trust and play a key role in supporting at the front door of A& E in preventing hospital admissions.
Utilising their knowledge of the community services and clinical skills the post holder will work closely with the A& E staff to ensure seamless, safe and effective discharge plans are in place for the person to return back to their usual place of residence as soon as possible, preventing admission to hospital.
The post holder will play a key role in using data from incidents, feedback and observation to identify potential improvements or changes within the service which will support effective safe discharge.

The post holder will have excellent clinical decision-making skills that will support the A& E staff in the management of discharging patients into the community and community beds.
To provide training and education to A& E staff around what support community services can provide and support in discharge management & processes.
To ensure records and data are maintained in a timely manner and in line with information governance and data sets required for the service.
To use clinical expertise to be able to support A& E staff in the patient pathway to facilitate rapid discharge and prevent hospital admission.
Professional qualification such as a degree or equivalent in Physiotherapy or Occupational Therapy.Current registration with the Health and Care Professions Council as a Physiotherapist or Occupational Therapist.
Evidence of relevant postgraduate courses / CPD and reflective practice relevant to clinical speciality
Training in student education
Experience of supervising junior staff, assistants and students
Knowledge of national policies and procedures relevant to the clinical setting
Knowledge of clinical governance, clinical audit and risk management
Supervision and appraisal of junior staff
Ability to travel
