Community services are changing- this is the perfect time to join a progressive and innovative team aimed at supporting the population of Guildford and Waverley.
Your focus will be on supporting a range of clinical services in community and you will provide both leadership, business and performance support.
This is an exciting opportunity to work within our Community Division supporting our multi professional teams to deliver our services.
You will work with the Clinical Service Managers and Head of Service to assist in furthering the development and delivery of all these services.
You will use your knowledge of leading and developing teams using a variety of data sources to support service function and development.
This post requires a person who has enthusiasm, experience of managing performance data as well as sound leadership and communication skills.
You will be required to implement change across the services and will need associated skills around service improvement and change.
You will need to identify and request good work flows to ensure performance information can be collected seamlessly, by working with our informatics support team.

We are clinically led and provide joined up care by bridging the gap between hospital and community services alongside regional specialist cancer care.
Our main acute hospital site is in Guildford with community hospital sites at Milford, Haslemere and Cranleigh. We provide adult community health services in homes across Guildford and Waverley.The Care Quality Commission (CQC) have given us an overall rating of Outstanding.

The post holder will develop and implement strategic improvement and delivery plans for the service portfolio .This will require leadership, facilitation and clear identification and direction on new or enhanced ways of workingThe role will involve collaborative working with partners as we develop a more aligned and integrated community health .
The management and leadership of these services will include responsibility for the management of a complex range of specialist multi professional services in an integrated care delivery approach with partner agencies.
A key element of this role is to lead the community division's performance .The post holder will have overall visibility and accountability for ensuring the services activity performance and governance agenda are met.
The post holder will need proven leadership qualities and will guide staff under their supervision through service improvement and delivery of the new pathways bringing credibility to the planning ambitions of the team.
Evidence of Team leader or management qualification or continuous management experienceMust have significant experience in operational management of service delivery
Requirement to have experience in managing, analysing and presenting data
Must have an understanding of the background to and aims of current healthcare policy/national guidance/CQC/ and appreciate the implications of this on engagement
Must have a good appreciation of planning and developing and implementing strategies
