Community Mental Health Nurse - Greenwich East ADAPT
We have an exciting opportunity for a nurse to join the Greenwich East ADAPT Team. The successful candidate will have the opportunity to work in a busy and varied multi-disciplinary environment. You will alongside clinical and other non-clinical colleagues to make a positive contribution to our patient's journey to recovery.
The Greenwich ADAPT Service provide care and treatment to individuals who are experience anxiety, depression, affective disorder, personality or trauma. If you are interested in developing your intervention based skills with this cohort then we would look forward to hearing from you and receiving your successful candidate will be supported to develop their assessment skills and psychologically informed interventions (both group and individual) to enable them to work as an effective member of the team.

Main duties of the job
The post holder will be responsible for undertaking comprehensive assessment of service users and carers mental health and social care needs. Once these needs have been identified the post holder will be responsible for developing a goal focussed and person centred care plan to enable the provision of an appropriate intervention.
The post holder will be working in a supportive diverse team, where there are opportunities for personal and professional development.

Oxleas offers a wide range of NHS healthcare services to people living in South East London and Kent and to people in prison. Our services include community health care such as district nursing and speech and language therapy, care for people with learning disabilities and mental health care such as psychiatry, nursing and therapies. Our multidisciplinary teams look after people of all ages and we work in close partnership with other parts of the NHS, local councils and the voluntary sector and through our new provider collaboratives. Our 4,300 members of staff work in many different settings including hospitals, clinics, prisons, children's centres, schools and people's homes. We have over 125 sites in a variety of locations across the London Boroughs of Bexley, Bromley Greenwich and into Kent and manage hospital sites including Queen Mary's Hospital, Sidcup and Memorial Hospital in Woolwich as well as the Bracton Centre, our medium secure unit for people with mental health needs.
