    We have a fantastic opportunity for a Dietetic Assistant Practitioner working in diabetes. Our community diabetes team is well established and provides high level virtual, face to face and group support for patients with Type and Type diabetes.

    This -month maternity cover post offers excellent opportunities with possible scope to support diabetes education including DESMOND.

    We have a well-established team of assistants and practitioners across the department, but this will be our first working in diabetes.

    The post holder needs to be a car driver.

    The Nutrition and Dietetic Service is a supportive team. We provide a service to a range of rural and urban areas across Cambridgeshire in a variety of settings including clinics (telephone, video and face to face), home visits and inpatient community hospitals. We also run several very successful groups both face to face and virtually as well as regular MUST training for care homes. The service has bases across the county and comprises Dietitians and assistants/assistant practitioners working in the speciality areas of Mental Health, Gastroenterology, Diabetes and Nutrition Support.

    Main duties of the job

    As a key member of the Cambridge and Peterborough Foundation NHS Trust, you will be joining an exceptional and well-regarded team. We require a motivated, compassionate, and dynamic person, with good communication and organisational skills.

  • To work within a multi-professional team in a community setting, providing, support, care, and treatment to individuals, within guidelines and protocols identified for the role in the specialist area of Adult Diabetes requiring first line dietary advice.
  • To assist in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community and in clinics (telephone, video or face to face as required), by following prescribed packages of care and pathways.
  • To support registered staff in the management and execution of clinical practice for patients with Diabetes
  • To carry out duties delegated and supervised by a registered professional.
  • To provide a high standard of care.
  • To manage own caseload of patients who require ongoing management and support with complex needs.
  • Wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure that patient receive the appropriate care in the most appropriate setting.
  • To Support diabetes group education.
  • This post can be based at any of our sites. There may be some flexibility around the number of hours per week for the right candidate though a minimum of hours/week would be required.
