    Clinical Lead for Accessible Care - London, United Kingdom - NHS South East London Integrated Care Board

    Job summary

    Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

    The immediate priorities for the postholder will be to scope a package of support, including training and practical advice for primary care providers to provide inclusive, compassionate care for patient groups who often find it challenging to access primary care and other upstream services; to provide clinical advice on delivery of the Health Inclusion and Proactive Care Frameworks; and supporting the development of wider new models to PCN working to support the sustainability of general practice.

    More generally, the postholder will act as a clinical champion for the further development of a consistent, personalised model of care, operated by INTs, in line with the key recommendations of the Fuller Report.

    Main duties of the job

    Clinical and Care Professional Leadership (CCPL) roles have been established in SEL ICB since April 2022 and have been a valuable source of clinical leadership.

    Within the Community-based Care (CBC) directorate, the role of CCPL for Accessible Care has an important remit, providing advice and support on complex system-wide transformation to ensure resident of SE London receive accessible care.

    The immediate priorities for the postholder will be to:

    o Provide clinical advice and support to reduce inequalities in access, experience and outcomes

    o Support the increase in sustainability of primary care

    o Increase health inclusion for groups that traditionally struggle to access care

    o Support self-care pathways and managing upstream interventions

    o Support the recovery of community waiting times through linking in with the Community Provider Network and regional forums

    The role will strongly link with the other CCPL roles within the CBC directorate and LTC-related CCPL roles at place and the postholder will be expected to make links with other LTC and Primary Care stakeholders at Programme and Place level.

    In agreement with the postholder, a more detailed workplan will be developed after appointment.

    The pay range for this role is £92,700 to £113,300 pro rata on 3 sessions per week, per annuum.

    Qualifications and Experience

    Essential

  • Qualified care and/or clinical professional NMC, HCPC, social work register, GMC etc. Educated to Masters level or equivalent level of experience of working at a senior level in specialist area.
  • Experience of transformational/innovative service delivery, productivity and/or transformation policy implementation.
  • Previous experience of being part of peer networks and collaborative working
  • Desirable

  • Good knowledge or cross professional transformation, digital solutions and innovative redesign.
  • Cross organisational and/or professional boundary collaboration experience
  • Knowledge and Skills

    Essential
