    Job summary

    The Enhanced Dementia Care Service is a true example of integrated working, being delivered in partnership with Hestia and Hounslow Health Primary Care Network. The service works with people living with dementia in Hounslow to enable them to live well at home, plan for changes in their health or wellbeing and prevent crisis.

    Successful candidates for this post will need to be self-motivated, forward thinking and committed to learning. You will proactively build professional relationships and work in collaboration with the multi-disciplinary team (MDT), patients living with dementia, their families and carers along with other members of the local health / support services.

    You must be flexible and responsive with excellent time management skills as the team is required to be reactive and priorities change frequently throughout the day.

    Main duties of the job

    We are looking for a confident and autonomous Occupational Therapist with relevant experience to work as the sole Occupational Therapist within a multidisciplinary team.

    You will be required to provide holistic clinical assessments as well asspecialist assessments of function and occupational performance to inform care planning.

    You will bring a comprehensive understanding of the role of Occupational Therapy in a community setting and within the context of Dementia. We are looking for someone who is enthusiastic and able to draw on their experience to deliver high quality clinical services to the population, whilst offering specialist input within the team. You will also bring an understanding of how to support individuals and their carers to plan for and prevent crisis in order to maintain quality of life and wellbeing at home.

    The service currently operates Monday to Friday 9am - 5pm but will be covering 7 days moving forwards. Applicants must be able and willing work over 7 days including extended hours and weekends as required.

    Qualifications and Knowledge

    Essential

  • Degree or equivalent in Occupational Therapy
  • Current registration with the HCPC
  • Evidence of postgraduate personal/ professional development
  • Experience

    Essential

  • Post graduate practical experience as a band 5/6 Occupational Therapist, including experience working with older people / people with dementia
  • Broad range of core clinical specialties with ability to relate to the advertised role
  • Desirable

  • Involvement in audit, research and understanding quality improvement process in the context of relevant national guidelines
  • Experience of supervising therapy assistants, junior staff or students
  • Skills and Abilities

    Essential

  • Ability to work as an autonomous practitioner to assess Occupational Performance in the context of Dementia
  • Understand and apply relevant local and national guidance to clinical practice
  • Understanding of clinical governance and its implications for services, including experience of quality issues and audit.
  • Work as an autonomous practitioner and have the ability to organise, prioritise and delegate work, in a timely fashion.

  • Experience of delegation to junior staff and assistants.
