Scale 29 - £37,336 to Scale 34 - £42,403 with a professional bar at Scale 31 - £39,186** Hours: 36 hours per week Base Location: Tameside General Hospital, Fountain Street, Ashton-Under-Lyne, Tameside, OL6 9RW Closing Date: Tuesday, 14th May 2024 Assessment Date: To be arranged The Role This is an exciting opportunity to come and work for Tameside Adult's Services on the Hospital social Work team, based at Tameside General Hospital. We are looking for a driven Social Worker to join Adult Services. If you are passionate about supporting adults to live well at home, we would love to hear from you. About you You will take the lead on supporting adults to live well at home. You will be part of a friendly and supportive team and your work will include managing a caseload, Care Act Assessments, Support Plans, Mental Capacity Assessments and Best Interest Decisions and signposting to internal and external services where necessary to enhance wellbeing and independence. You will have excellent communication skills and have the ability to build effective professional relationships with other disciplines and with other local health and social care services to enable a coordinated response to individuals and families.
