- Support the Maternity Clinical Risk Manager to co-ordinate patient safety and clinical governance activities within the maternity department. The main responsibilities will include reviewing case notes, investigating incidents and near misses and analysing themes and trends from incidents and near misses.
- Support and provide guidance to all staff within maternity services in relation to patient safety and clinical governance
- To contribute to the maternity clinical risk management strategy. And provide support in monitoring the effectiveness of the maternity services risk management strategy.
- to provide feedback on incidents and progress the implementation of recommendations from multidisciplinary meetings.
- Work with the lead clinicians for clinical risk within maternity, Supervisors of Midwives, Midwifery matrons, SWSH Service Manager, Medical and Midwifery staff to identify clinical risk issues and implement changes in practice.
- Promote and maintain cohesive working relationships with obstetricians, neonatal team (medical and nursing) anaesthetists and other specialists.
- Support the implementation of recommendations from Government and professional bodies in relation to maternity care.
- Maintain a clinical and leadership role.
- Conduct risk assessments and suggest and influence changes in practice.
- Lead on, identify and support appropriate staff with investigations of clinical incidents, adverse events and serious untoward incidents in a timely, non-judgemental and comprehensive manner.
- Review clinical incidents which have been reported via the Datix database system and ensure investigation and escalation where appropriate. Ensure that staff receive appropriate feedback following the investigation of an incident.
- Attend the weekly CLIP meeting and feedback on the actions taken following incidents and investigation progress.
- Maintain effective channels of communication to feedback to staff the outcomes of clinical incident investigations
- Co-ordinate meetings with women and their families and any external agencies affected by adverse incidents.
- Co-ordinate round table meetings to facilitate timely debriefing and investigation of incidents and clinical near misses.
- NMC Registered Midwife currently registered to practice
- Evidence of Post Registration
- Education; ENB 997 or equivalent
- Ability to think and practice at Masters level
- Relevant Degree
